Stroke is the fourth leading cause of death in this country. Stroke occurs when the flow of blood carrying oxygen and nutrients to healthy brain cells is disrupted, due either to a vessel bleed or a blockage within the blood vessel. If the blood supply is not quickly restored it would lead to death of the brain cells (an infarction) that are supplied by the affected vessels. Ischemic and Hemorrhagic Strokes The two major types of stroke are identified as hemorrhagic (due to a bleed), and ischemic (due to a blockage). The large majority of strokes are due to ischemia resulting from a blockage in the vessel and can be treated by dissolving or removing the blood clot or thrombus and restoring blood flow. Time is of the essence when relieving the blockage, which is why stroke centers coined the phrase "time is brain" when referring to stroke treatment. Most legal cases arise when the stroke is not promptly diagnosed or treated in the emergency room, costing the patient a significant opportunity for a good outcome. If one can relieve the blockage before permanent damage occurs, significant disability can be avoided. Treatment of Ischemic Strokes The standard treatment to relieve a clot or a thrombus is the administration of a "clot-busting drug" known as tissue plasminogen activator or "tPA" which can be administered intravenously for 3 to up to 4.5 hours after symptom onset. If the patient arrives to the emergency room more than 3 to 4.5 hours after symptom onset, there may be advanced treatment options that are available to dissolve and/or remove the clot including: 1) The administration of the clot busting drug "tPA" through a catheter directly to the clot; 2) The use of mechanical devices to remove the clot; or 3) A combination of these treatment approaches. The accepted time windows for these advanced treatment options range from 6-8 to up to 24 hours after symptom onset, depending on the location of the blockage and other factors. With all of these treatments, the earlier the treatment is administered, the better the chance of avoiding significant disability. Treatment of Hemorrhagic Strokes This type of stroke, accounts for perhaps only fifteen to twenty percent of the strokes, but can be similarly devastating as brain tissue is deprived of blood supply due to the "rupture" of the artery carrying blood to the affected region. Once a rupture occurs, timely diagnosis and emergent intervention by a well trained and experienced neurosurgeon or interventional neuro-radiologist, is needed. At times, depending upon the circumstances, techniques less invasive than open surgery can be successfully used to treat. At times, these strokes are preceded by a complaint of the onset of "the worst headache of my life," and an expansion of the wall of the artery can be discovered radiologically before rupture allowing, at times, for treatment to be offered before bleeding occurs. Failure to Treat Signs and Symptoms of Stroke On a Timely Basis Most medical negligence claims involving stroke revolve around the failure to diagnose and/or treat the signs and symptoms of a stroke on a timely basis. Over the past several years more and more hospitals have applied for certification to become either a "primary" or "comprehensive stroke center", requiring that the hospital follow the guidelines of the American Heart Association and the American Stroke Association (AHA/ASA) for treating stroke. Failure of a hospital certified as a stroke center and its stroke team to work up and treat its patient in accordance with the AHA/ASA guidelines may form the basis for a claim of negligence and the breach of standards. Even at hospitals that are not certified as stroke centers, certain standards must be met in recognizing the signs and symptoms of stroke and acting promptly to work up and either treat the eligible patient or arrange for transfer to another hospital that can provide the necessary treatment under accepted medical standards. Not only can the doctors involved in the stroke care be held responsible, but the hospital itself can be held to account for its failure in having the necessary facilities, equipment, policies, and staffing in place to assure prompt and proper stroke care. With the availability of telemedicine and interactive video access to specialists at "comprehensive" centers capable of providing the most advanced stroke treatments, there is little basis for failing to offer patients potentially life-altering stroke care to eligible patients at least by transfer to another hospital. Proving a Causal Connection in the Stroke Case As with all medical negligence cases, proving substandard or negligent care is only half of the battle. What must also be proved in each malpractice case is the causal connection between the claimed negligence and the harm. It is the person bringing the case who must prove this causal connection in each case. Where the underlying condition (that is, the clot or blockage which precipitated the stroke) pre-exists the negligent care, one must show that proper diagnosis and timely treatment would likely have resulted in a significantly better result. Given the risks associated with treatment, and the fact that treatment will result in a good outcome in only a certain percentage of cases, establishing the likelihood of a good or significantly better outcome from what is a serious medical condition is the most difficult part of proving the stroke case in court. The material that you need to use should measure between eight and ten inches square. Here are 6 tips I use for friends, families and clients determined to get the most from their National Park experience. #1 Tip Number one is identify from where you want to visit the Canyon. There are two key starting points: Las Vegas and South Rim. From Vegas you'll have your choice of West Rim and South Rim tours. If you are starting out from Arizona (key cities include: Phoenix, Scottsdale, Sedona, Flagstaff, Williams and Tusayan), you'll want South Rim tours. #2 My second tip is determine which rim you want to visit. As noted, there are two: The West Rim and the South Rim. The West Rim is 120 miles from Las Vegas, NV, and tours there originate primarily from Vegas. The South Rim, which is located in northern Arizona, is usually a destination for AZ-based travelers. Fact is Park tours are popular and they sell out all the time. To avoid this, I recommend reserving your tour at least a week ahead of time. If you're starting out from Las Vegas, I'm going to advise two weeks (especially when it comes to helicopter rides). I've learned over the years that immediately after you book your hotel you book your tour and it's a system that works. #4 The fourth tip is to book online. I could write a book on this and of course my thoughts on this are always changing. So here's where I stand now: First off, the best deals are on the Internet. For the most part, you will get wholesale rates. And you won't get slapped with a booking fee. #5 Tip #5 is understand all the above and then select a tour that falls within your budget. Helicopters are the most expensive and bus tours are the cheapest. Airplane and rafting tours are in the middle. In regards to air tours, you have air-only and landing. Flights that land are only available from Las Vegas. Further, helicopters that land at the bottom of the Canyon ONLY leave from Las Vegas and ONLY go to the West Rim. #6 My final tip is understand the cancellation policy. This is something I can't overstress. Further, if you purchase your trip from a vendor other than the ones I endorse or work with you are on your own. If you do go with any of the companies associated with us, you're covered in this sense that you can cancel within 48 hours for all your money back. And for those looking to really impress a loved on, a helicopter tour over the Grand Canyon is a great option. Just make sure you book ASAP as these "sweetheart" flights sell out quickly. Las Vegas From Vegas, you fly to the West Rim, which is just 45 minutes by air. Types of tours range from air-only to landing, and both kinds fly over Lake Mead and Hoover Dam en route to the National Park. The air-only flight is perfect for folks who have limited time to sightsee the Canyon. It's also the cheapest option and perfect for those on a budget. That said, I prefer landing tours because they offer you a much more comprehensive experience. Specifically, I like the landing tour that goes to the bottom for a Champagne picnic. And for those feeling especially adventurous, I recommend extending this trip with a Colorado River boat ride. Whichever you go with, you're in for one heck of a trip. Deluxe Flights Because Valentine's Day is so special, I'm also recommending that you consider taking a deluxe flight that departs from the Las Vegas Strip. I say this because they come with free hotel pick up and drop off by limousine as well as an extended flight over The Strip on the way back from the Canyon. Further, by going deluxe, you're assured of getting onboard an EcoStar 130 helicopter. These particular aircraft are the best sightseeing choppers on the planet. This is because they come with 180-degree windshields, stacked stadium-style bucket seats and an aerodynamic design that makes the ride super smooth. South Rim Here you get to choose between 30-minute and 50-minute flights. The shorter one gets the job done but if you're doing this for Valentine's Day, I recommend going big and taking the 50-minute ride. This particular tour covers nearly 75 percent of the entire Park and includes the Dragoon Corridor, the widest and deepest section of the Canyon. Another South Rim trip I like is the helicopter/Jeep combo. It's the one that features a 30-minute helicopter ride over the Canyon that concludes with a 3-hour ground tour inside the National Park via a customized Jeep. In fact, this one gets two thumbs up from me because no other tour delivers the types of aerial and ground views you'll get on this trip. Dress Accordingly Bear in mind that February is one of the coldest months of the year at the Canyon. My suggestion is to check out the weather forecast a couple of days prior to your flight and dress for it. Temps at the West Rim are similar to those you'll get in Vegas. The South Rim sits at 7,000 feet so if you're coming up from Sedona or Phoenix you'll want to bundle up. Quite simply, it is the thing(s) that make your business better and give you an advantage over your competitors. It is the 'differentiator' that will help to ensure your business thrives. Ask yourself, what makes my business better than that of my competitors? What sets us apart? These are the fundamental questions that you should be asking yourself as you seek to innovate your business processes and attract more customers with the aim of not only reaching your short term goals, but more importantly, sustaining your business over the long term. "Your competitors never stand still, so neither should you!" As in all areas of life, business and markets are fluid and dynamic things that are constantly changing, so deserve your constant attention. What set Nokia apart as the world-leader in mobile phones was blown out of the water when Apple introduced the game-changing iPhone back in 2007. Remember, your competitors never stand still, so neither should you! Ok, so I know what competitive advantage is, but how is it achieved in the real world? In reality, lots of different ways, Davidson in his seminal work Even More Offensive Marketing (1997) identified many types of competitive advantage, here are seven of them: 1. Having a superior product or service. 2. Having a perceived advantage in customers' minds through advertising and brand positioning, for example, think Coke versus Pepsi. 3. Cost leadership. This isn't just about being the cheapest, it is ensuring your overhead costs are streamlined, which in itself can have a significant effect on profitability, and remember, turnover is vanity, profit is sanity. 4. Legal advantages such as copyrights, patents, sole distributorships etc. making it difficult for your competitors to 'muscle-in' on your business. 5. Superior relationships with customers, with suppliers, distributors and partners. 6. Possessing superior skills and competencies. 7. Attitude advantages. Are you happy where you are or are you eager to drive your business forward? Take a long look at the above and ask yourself, how am I achieving competitive advantage in my business? Are there things I could be doing differently, or better? Why would my customers choose my business over my main competitors? What makes my competitors good/bad? These types of thought provoking questions will prompt you to think about your business in a way that embraces the concept of continuous innovation, continuously reinventing your business, building on previous successes and seeking new, exciting ways to succeed again and again. Therein lies the crux, competitive advantage on its own is not enough, you need to constantly strive to innovate, reinvent and change in order to achieve 'sustainable competitive advantage', the holy grail of achievement for all market economy businesses. Imagine Apple still trying to sell the first generation iPhone today versus the other smartphones on the market, or Microsoft trying to pit the first generation Xbox against Playstation 4.
