    Emergency medicine physicians must complete rigorous training, just as all physicians do. Interestingly, however, the American Board of Emergency Medicine (ABEM) was at one point granting board certification to non-emergency medicine trained physicians if they had already spent a certain number of hours working in an emergency department (ED). This period lasted from 1979 to 1988, but after that time, the policy was changed so that only physicians who completed an emergency medicine residency were allowed to receive the designation of being board-certified. After 1988, more board certified emergency medicine physicians were sought after and hired in hospitals around the United States as the medical community recognized the benefits of employing specialists who have this training. Surprisingly however, this was not the case in Veteran's Affairs hospitals.
     
    The Veterans Health Administration (or VHA) is part of the United States Department of Veterans Affairs, and it administers outpatient clinics, hospitals, medical centers, and even nursing homes for veterans. For reasons that are unclear, emergency departments at VA hospitals did not respond to the changes in emergency medicine education; in 1993, only 26.1% of surveyed hospitals run by the VA were seeking to hire board-certified emergency physicians. A more recent study completed in 2009 sought to examine whether this was still the case, and to explore possible changes that have occurred over the past 15 years.
     
    To complete the study, 120 VA hospitals were surveyed. Surveys were sent to the section chiefs of each hospital, and only those hospitals with fully functioning emergency departments were selected for participation. To help ensure that the data were interpreted accurately and without bias, all statistics were analyzed by an independent statistician with no knowledge of the purpose of the study. The authors sought to find whether any statistically significant changes had occurred since the original 1993 study that examined emergency departments in VA-run hospitals. That is, they sought to determine how the numbers of emergency medicine residents and physicians had changed over the past 15 years and whether these changes were statistically significant.
     
    The results of the survey study were somewhat encouraging. All 120 hospitals employed a full-time physician all days of the week and all 24 hours per day; these attending physicians were either board certified in emergency medicine, internal medicine, or family practice. 65 out of the 120 hospitals (65%) employed either a full-time or part-time physician who was board certified in emergency medicine. 45% of the hospitals were thus employing physicians who were certified in either internal medicine or family practice, but not emergency-medicine. Only a small portion of the facilities that were surveyed (7.5%) had hospital coverage that was provided only by emergency medicine residents (rather than physicians who were board-certified). In contrast, 51.7% of the hospitals had coverage that was only provided by attending physicians (ie those board-certified in internal medicine, family practice, or emergency medicine) without residents. So regardless of their specialty, most hospitals at least had board certified physicians, and 65% had at least one physician certified in emergency medicine.
     
    The authors point out that the growth in the numbers of emergency-medicine certified physicians is at the very least promising, but the numbers of residents employed at VA hospitals have not grown significantly. The reasons for this are not entirely clear, but one good point that the authors bring up is that if more emergency medicine residents are given the opportunity to complete their residency training at VA hospitals, they will be better prepared for the unique challenges and expectations required when treating patients at a VA hospital, and possibly more likely to eventually choose to work in a VA hospital. The authors suggest that emergency medicine programs should strive to partner more frequently with VA hospitals and health centers in the future.

    There are two major types of tours to the Grand Canyon. Grand Canyon air tours is one, while the other is a ground tour usually conducted by a bus or shuttle vehicle. Flights usually depart either from Las Vegas or from the National Airport at Grand Canyon, AZ. Understanding what it is you want to learn while you are visiting this part of Arizona will help you determine which type of tour you should book for yourself.
     
    When you visit the location by air, the vast expanse and grandeur of this area is obvious. The size involved is eighteen miles wide at its maximum point and reaches a depth of about 6,000 feet. Its length is 277 miles. It is impossible to realize the scope of these measurements from the ground, even from one of the several famous vantage points.
     
    Views by air give the visitor a better grasp of all the colors that can be seen. The water of the river itself can vary from rapid whitewater to the deep blue colors of Horseshoe Bend. The rocks vary from orange to tan to brown to red, depending upon the layers of sandstone that have been worn down over hundreds of years. Of course, the desert colors and the bright pinpoints of clothing and decorative areas from people on the ground add interest.
     
    Although you miss the "feet-on-the-ground" close-ups of the chasm, you will get to see much more by flying over the expanse. Many of the flights allow for views of other areas such as the Painted Desert and Hoover Dam. These elements add to the interest and features that can be accomplished in a single trip.
     
    Overall, taking a plane over the area is the most comfortable way to see the sights. You don't have to spend hours walking or riding in order to cover a lot of territory. The seats are comfortable and the windows allow for the best opportunity of seeing the sights.
     
    Although photographic opportunities abound throughout the canyon tours, by using the air viewpoint, there are more opportunities to use light, color and vistas to construct scenic images. It is said that no two pictures of the canyon are identical. The variety of shadow and light during different times of the day makes for an ever-changing opportunity to capture unique images.
     
    An important benefit of using air rather than ground-based tours is the time required. You can set aside a day for viewing the West Rim or the South Rim and have time left over to relax with a meal during the tour. On the ground, you would see only a point or two up close.
     
    Choosing Grand Canyon air tours over ground excursions will be a good choice for people who want an overview of the area. You will be comfortable while you see wide horizons. If you prefer a close up view of cultures, or if you want to enjoy adventure, the ground tours will be more to your liking. In either case, the view will be sure to meet your expectations.

    One of the most important aspects of a successful wellness programme is to maximise employee participation. And, one of the most crucial ways to maximise employee participation is to ensure that the program is fun! However, recent research shows that companies often overlook the need for fun.
     
    Recent research examines fun at work in global company wellness initiatives
     
    Changing health behaviours for the long-term is critical to reducing the developed world's soaring obesity rates, incidence of chronic disease and their fiscal and cultural impact to organisations. A recent research report by GCC published in 2013 provides insight into how organisations are approaching long-term health behaviour change and highlights the key successes and challenges 378 organisations face in the workplace today. It examines company priorities and where programme design is letting them down.
     
    Fun is essential to maximising participation in workplace wellness programmes
     
    Arguably, the most important aspect of a successful wellness programme, is engaging staff in the process. Maximising employee engagement and participation is crucial. Without this engagement, there is little point in running a programme. And, leading workplace wellness providers have found that employees are more likely to want to join an initiative that is fun.
     
    It takes time to change lifestyle behaviours for the long term; therefore enjoyment goes a long way towards supporting strong completion rates. This is why some experts argue that a key performance indicator for wellness initiative success, in enjoyment. However, 'fun' is not always what happens.
     
    Employers understand the need for corporate wellness fun
     
    In general, companies do seem to understand the need for corporate wellness fun. The GCC study found that 99% of organisations rate the importance of having fun at medium or higher in regards to their wellness initiatives. However, whilst 37% of organisations report having fun as a very high importance for wellness initiatives, only 10% actually report their initiatives achieving very high levels of fun.
     
    How to make sure your wellness initiative is fun!
     
    Wellness initiatives can often be perceived as dull - and many inherently are. The ability to create an initiative that is viewed as fun is critical increasing participation rates and, ensuring good completion rates - both prerequisites for achieving long term behavioural change amongst employees.
     
    Here are some ideas to ensure maximum fun and participation in your wellness programme:
     
    - Ensure careful planning and informed design – start by asking, 'how can we make this programme fun for our staff?' Be aware of the workplace demographic, ask staff what they want, what would motivate them and involve staff in design
     
    - Commitment from senior leadership – commitment to a 'fun' focus needs to be fully embraced by senior leadership
     
    - Create a good champion structure – ensure a strong champion structure, which is adequately resourced (including being given adequate time)
     
    - Provide enough time - 86% of respondents in the GCC study reported lack of time being the main reason employees did not take part in wellness initiatives. Creating a strategy that breaks down this barrier (or 'excuse' as some may say) is critical to ensuring high uptake of wellness initiatives.
     
    - Regular review – review the programme regularly to ensure that it is still relevant and fun
     
    - Communications – organisations should communicate their programmes and the reasons for them thoroughly and regularly. Employees need to know that the programme will be fun in order for them to want to be involved!
     
    - Incentives – Participation in programmes with explicit incentives can attract 85% participation or better. Incentives can include both financial and non-cash rewards ie gift cards, merchandise, travel, time off and praise. Fun is a big player here too!
     
    Conclusion: Never forget the need for fun at work for maximum corporate wellness!
     
    The GCC report reminds us of the need to be vigilant and enthusiastic when it comes to promoting employee wellness initiatives that are fun, fun, fun at work! Make fun your starting point, rather than an afterthought, in order to ensure maximum employee participation in your wellness programme.

    Traditional stress research has tended to focus on the things that we can do when we get stressed. However, latest coping research suggests that there are some things that we can do before stressful events occur, so that we can cope better when stressful events occur. As a result we will smile more and stress less!
     
    Traditional stress strategies
     
    Traditional stress management strategies focus on what we could do when life becomes stressful. They include things such as planning, building physical resilience, improving leisure behaviour and making changes to the work environment. For instance, we might learn to plan ahead so that we can reduce the number of things that we need to get done in any one day. We can make sure we get a good night's sleep, exercise sufficiently and eating well. There is often a focus on developing work-life balance, making sure that we learn to relax and have some fun. Finally, these strategies might involve discussing undue work stress with management and possibly reducing one's workload. Traditional stress management strategies are very useful when undue stress enters our lives. But is there anything that we can do to prevent stress before it happens?
     
    New proactive coping strategies
     
    Latest coping research argues that it is not enough to merely be armed with coping strategies, should an untoward event occur in our lives. Unplanned and potentially stressful events will happen. Therefore, we can be proactive and prepare in advance. If we lose a job, there is a death in the family, or we injure ourselves for instance, we will be better equipt to cope.
     
    Alot of our ability to cope has to do with our attitude. If we see an event as awful and something we won't be able to cope with, then we will feel overwhelmed and very stressed. However, if we see an event as something that will teach us life lessons and help us to grow, than we will feel less overwhelmed. Here are some strategies that you might want to try:
     
    - Develop a positive spiritual or philosophical approach to life: Acknowledge that life can be challenging. That is 'just life!'. If we do this, rather than feeling merely hard done by when something goes wrong, we will challenge ourselves to stay positive and do the very best we can for ourselves and others.
     
    - Establish important priorities: Knowing which things are important to us in life, can help us make positive decisions when things go wrong. In other words, we know clearly where to put most of our time and energy and which things we need to let drop off.
     
    - Seek personally meaningful goals: This links to the point above. Set personally meaningful goals in all areas of life including family, health, spirituality, work etc. This keeps us motivated throughout the good and not so good times.
     
    - Maintain some reserves: Knowing that we have very little lee-way in our life if things go wrong, can create a lot of stress. So, put aside some finances 'in case' you need it and make sure that your life is not so crammed full that you can't take some time out to cope, should an emergency arise.
     
    - Plan proactively: Plan your life well so that you will not be in reactive mode, but can take the time to think through a problem, should it arise.
     
    Don't let stress rob you of your joy!
     
    Stress has a habit of robbing us of our joy and sense of fun in life. When we feel stressed, we can feel overwhelmed and worry that we cannot cope with everything in our life. However, if we plan proactively, we will have the tools and resources to not only cope, but to grow and even thrive as we grow through the tough times. In other words, we will be stressing less and smiling more!

    We tend to think that when we are busy we are achieving more. However recent research suggests that 'being busy' may have the opposite effect to what we intend. Being busy can leave us unsure of our priorities, block creative thinking, and generate more stress and ill-health. We may be 'doing more', but what we are doing may not be helping us to achieve key goals. If we slow down, we can become more focused on key priorities.
     
    1. Make careful leisure choices
     
    In affluent Western society, we have many choices about how we can use our time. We often feel time poor. However, research shows that people in the US have more leisure time than they did in the 1960's. These days we can choose to fill our leisure time with a whole host of activities including exercise, socialising and education. Sometimes we feel overwhelmed by these choices, and wonder how we are going to fit in everything we want to do! Rather than trying to do everything, we need to be strategic about how we use our time, maintaining a strong sense of priorities and making careful leisure choices.
     
    2. Don't equate work with stress
     
    These days, many people simply equate work with high stress. It is true that in the technological changes have irreversibly changed the nature of work. With the advent of email and smart phones, work can be delivered to our inbox easily. Given the instant nature of communication, there is an expectation that we respond to colleagues and clients fairly instantaneously. However, the most effective business people will tell you that it is still necessary to stay in charge of and prioritise one's workload. Do not be controlled by others demands!
     
    3. Don't confuse busyness and stimulation with quality of life
     
    As a result of television and the internet, we can receive new information, entertainment and high stimulus at any time during the day. However, medical researchers point out that high constant stimulation is not good for our health. Constant stimulation can lead to chronic stress related illness such as heart disease and diabetes. Moreover, busyness can impact our ability to have fun and enjoy life. As is becoming evident with many children today, constant stimulation can dull our capacity to enjoy the small things in life. So, remember to stop and smell the roses!
     
    4. Don't confuse busyness with productivity
     
    It can be easy to feel noble when we are busy. If we are busy, we imagine that we must be 'working hard' and are therefore being very productive. However, in actual fact the opposite can be true. Research has shown human innovation and creativity needs space and time to flourish. It is therefore necessary to maintain a good balance of activity and rest in order to be productive. If we are too busy, we fail to be creative and our lives can be likened instead to a mouse on a treadmill. Let's not forget the power of the day-dream, leading to the space and capacity to imagine different possibilities!
     
    Conclusion – take back control!
     
    These four tips all come down to taking back control of our life and time. In a busy, over-stimulating world, this can be difficult. Yet, if we take the time to sit down and map out our priorities, we will be well on our way. If we stick to our priorities, we may no longer be feverishly busy, but we will be happier, healthier and more truly productive.

    We often see happiness as an end goal, something that we have to work towards. When we have everything we want: money, success, a family, a face lift!, then we will be happy and feel complete. However, recent gratitude research has turned that understanding on its head. Rather than working towards feeling happy and complete, research shows that gratitude may instantaneously provide the very things that we seek. Starting the day deliberatively grateful and, practicing gratitude throughout the day, has been shown to have a significant impact on both our health and happiness.
     
    What are the benefits of gratitude?
     
    Research has shown that people who regularly practice gratitude experience a range of health and wellbeing benefits. In other words, they experience the happiness that they seek! Such people tend to:
     
    - Exercise more
     
    - Have more energy
     
    - Be more alert
     
    - Sleep better
     
    - Experience less stress
     
    - Have a wider range of positive emotions including happiness, joy and fun
     
    - Have a stronger sense of priorities, with less focus on material wealth
     
    - Have more active social networks
     
    - Are more able to handle stressful situations
     
    - Experience lower levels of depression
     
    Start every day with a heart full of thank-you!
     
    Gratitude is a 'heart' attitude. However, it can take a little work. Let's face it, we don't always feel like being grateful when the alarm goes off in the morning! It is therefore useful to see gratitude as a daily practice. Here are some tips on turning your heart into a grateful, pumping organ:
     
    - Visualise all the things you are grateful for: Consciously be thankful for anything you can think of. I like to visualise my life in a series of concentric circles that start with me and radiate out to the wider world. For instance, I start with the little things ie a good night's sleep, clothes to wear, food on the table. Then, I move out from there to be grateful for my personal relationships, my job, the beautiful country I live in.
     
    - Keep going until you feel grateful!: Gratitude doesn't always come naturally. So, if you don't feel grateful, keep going until you do. Smiling a little to yourself 'as if' you are happy can also help to shift your mood from a grumpy one.
     
    - Be grateful for your future happiness: It is also useful to be grateful for the things that you hope to have in the future. Years ago, I started every morning by being grateful 'in advance' for my lovely husband, my house, my career and my little girl. At the time, I had none of those things. However, being grateful for them in advance kept me focused and now, I have them for real!
     
    Conclusion: Experience happiness now!
     
    Integrate the practice of gratitude into your everyday routine and way of thinking. The amazing and crazy thing about gratitude is that we don't have to put off being happy until some grand point in the future! Rather, by being grateful for all the blessings we have now, however small, we can find both health and happiness right now.

    We tend to think of work as a place where we need to be intensely active. And, these days workers are often expected to perform at a constantly high level. However, there are benefits to down-time at work. Down-time does not necessarily mean being lazy. It merely means recognising the human need to keep a balance of intense activity and relaxation, for good health and productivity.
     
    Maintain a stress-relaxation rhythm at work
     
    There are certain times in the work-place where we need to work very hard in order to complete a project. A company may understandably expect staff to pull out all stops to meet a deadline. Stress is a physiological response that can help us achieve this kind of goal. The 'fight or flight' response sets up a chain of physical reactions in our body, which readies it for intense activity eg faster breathing, pupil dilation, adrenalin. This can be a positive and useful mechanism to help us get something done.
     
    However, once we have finished an intense piece of work, we need to let the body enter into a relaxation and recuperation phase. In other words, we need to let the body, mind and spirit heal and recover. As a result, the body moves back into a position of natural homeostasis. It is then resilient enough to cope with stressful events once again.
     
    It is impossible for the human body to continue at the same level of stress and stay healthy. If we push ourselves and do not allow for relaxation, we will become chronically ill. In other words, we are doing no one, including ourselves a favour, by working like a dog!
     
    It is necessary for those in management roles to recognise that staff need to enter a relaxation phase once they have completed an intense piece of work. This does not necessarily mean that they will be doing no work, but rather they can do work at a slower pace, or less intensely demanding work, for a short time.
     
    Relax and get more done
     
    When we are stressed we are less able to concentrate. Chronic stress, leads to ill-health and ultimately, burn-out. It is very important to take tea breaks and lunch breaks. And, to stop regularly every 40 minutes. If you find it hard to stop, ask yourself: 'is what I am doing really that important?'
     
    Finding ways to relax at home and work
     
    Because of the busy lives we lead, in can be difficult sometimes to find ways to relax. Firstly, we often feel we don't have time. Secondly, because we are used to high stress levels, we often find it hard to relax. Here are some things that can help us shift into relaxation mode:
     
    Stress less at home
     
    - Gentle exercise such as yoga or tai chi
     
    - Massage – a good reminder to the body of 'how' to relax
     
    - Ask yourself what relaxed you as a child and start doing those things again
     
    - Be disciplined about stopping when you need to – don't try and be a saint!
     
    - Laugh eg see a funny movie – the relaxation response after good belly laughter is a powerful one
     
    Stress less at work
     
    - Don't eat your lunch at your work desk – it is not possible to properly digest food if you are in fight or flight mode
     
    - Maintain good social relationships with colleagues who you can relax and have fun at work with
     
    - Install micro-break soft-ware on your computer
     
    - Suggest some good fun at work activities to get people away from their computers eg celebrating birthdays
     
    Relaxation is gold!
     
    Relaxation can feel like 'laziness' in our busy world. However, it is actually a key to balance in life and is a wonderful gift. With balance we are fully able to concentrate and also appreciate everything that we have. Try then, to value the power of relaxation and you will reap the rewards.

    Evaluation: the massive trace that digital media leaves every time there's access to a platform or website means there is an opportunity for both qualitative and quantitative analysis and evaluation of digital marketing activities. This gives room for optimization of the tactics for better improvements.

    The game of boules, otherwise known as pétanque, is perhaps the sport that is closest to French hearts. Similar to British lawn bowling or Italian bocce, the French version is traditionally played with metallic balls on a dirt surface beneath plane trees, with a glass of pastis at hand. The object of the game is to throw your balls — usually with somewhat of an arched back-spin — so that they land closer to the small object ball (cochonnet) than those of your opponent, or strike and drive the object ball toward your other balls and away from your opponent's.
     
    It is difficult to provide a precise list of the types of boules games, because as demonstrated above the same game can be known by different names in different languages and locations, and because the same name can be used for different local variations of a game. I guess you have to turn up and watch for a while to see if the game being played bears any resemblance to the game you know.
     
    The game seems to have originated in ancient Greece and at that time they would certainly only played with stones - thankfully things have moved on since them. Mind you stones are cheaper!
     
    Where can I play?
     
    Similar to the local boulodrome which is a social focal point in southern France, Nerja has an abundance of areas dedicated to Boules and Petanque.
     
    Notable areas are at the seafront by Torrecilla beach at Plaza Congrejo and by the Supersol supermarket opposite Hotel Nerja Club right next to the skate board park although you will find that many of the urbanisation throughout Nerja, certainly the larger ones will have a Boules court tucked away somewhere. I have seen them at Capistrano Village, Verano Azul and many other urbanisations but for some reason they seem to be less active and most a bit over grown. I am assuming that because of security, guest players perhaps are not welcome as much in the actual urbanisations. If you are staying in a rental villa in nerja then you can always ask to use one, I see no harm in asking.
     
    Whilst you do not see much action during the summer months the sport takes a new lease of life during winter being a very popular pastime for visitors on long winter lets or just holiday makers staying in an apartment rental nerja and the expat community. The gathering can be quite large, 50 or so I have seen at times may be more either taking part or just onlookers.
     
    How much does it cost?
     
    As far as I can see - Nothing! All you need is to turn up with a set of Boules which can be purchased in many shops in Nerja and a sneeky bottle of Vino (rules of the game I think) and your away!
     
    There is a local club where you can learn about the sport and join in a coffee with others like minded players. There is also a series of competitions throughout the year and the social scene includes organized dances and quiz nights.

Walking is the simplest thing you can do for yourself. Walking will improve lung and muscle function. Your heart will love you for it. Carry a water bottle to remain hydrated particularly if it's hot outside. You can sweat out a substantial quantity of water and nutrients in thirty minutes if you are not careful.
     
    Losing weight takes commitment to yourself however staying motivated can be a challenge sometimes. If motivation to lose weight was straightforward to come back by, nobody would be overweight in our country. We would all be our ideal weights and have no troubles.
     
    It's up to you to figure out what things can help you get and keep motivated. Everybody is aware of the consequences related to having too much body fat and living a sedentary lifestyle. It's one of these things though that until it happens to you or somebody you love, you only keep complacent and suppose that it will never happen.
     
    After all, it can get out of hand if don't take management now. There are a few aspects to losing weight, like eating better foods, less junk, and getting a ton of exercise. Believe it or not, for several individuals the motivation to lose weight will usually start with finding something you can stick to. That's why I counsel starting with walking for thirty minutes a day. It's straightforward and easy to stick to. You'll perpetually incorporate more into a work out, but it pays to begin simple.
     
    Another nice manner to induce and keep your motivation is to find a like minded friend to exercise with. It can be as straightforward as the 2 of you taking a class together, joining a gym, or simply going for walks, having a buddy can make all the difference.
     
    One thing you don't need to try to to is place yourself below a ton of pressure by putting in place an unrealistic time frame. Don't wait till a few weeks before a marriage, or vacation. If you are doing that, you'll just be setting yourself up for failure.     What is Sciatica & What Causes it?
     
    Sciatica is a set of symptoms that occurs when the body's largest nerve (the sciatic) is impinged by a misaligned, inflamed, or otherwise damaged portion of spinal anatomy. The sciatic nerve originates in the lower back, which just happens to be the region of the spine most susceptible to degenerative changes, because it supports a bulk of the body's weight and facilitates a great deal of movement. For this reason, sciatica commonly occurs as a result of the natural aging process and the toll that time has taken on the spine.
     
    What are the Symptoms of Sciatica & How is it Treated?
     
    Many patients with sciatica experience radiating pain, numbness, tingling, and muscle weakness along one side of the lower body (the hip, buttock, thigh, calf, and/or foot). Treatment – at least initially – focuses on relieving these symptoms through nonsurgical means. Every patient's physician-recommended treatment regimen will vary, but some of the most common conservative treatments for sciatica include:
     
    Medication – Over-the-counter, nonsteroidal anti-inflammatory drugs (NSAIDs) like naproxen and ibuprofen are often the go-to drug because they're relatively safe, inexpensive, easy to find, and fairly effective. Patients with certain cardiovascular or gastrointestinal conditions may be advised to take an analgesic like acetaminophen instead. If over-the-counter medications prove ineffective, some physicians may prescribe narcotic pain relievers and/or muscle relaxants.
     
    Hot/cold therapy – Applying heat to the lower back can relieve muscle tension and increase the flow of nutrient-rich blood to the area. This can potentially relieve neural compression and might even aid in the healing process. Applying ice, on the other hand, can reduce inflammation and numb pain. Some patients may be advised to use hot and cold therapies alternately while others may be instructed to only use one or the other exclusively.
     
    Weight loss – Overweight patients can sometimes benefit from losing weight. Because the lower back is already tasked with supporting a great deal of weight, excess pounds can really affect spinal health. It should be noted that losing weight won't solve every patient's problems, and no one should begin any weight-loss journey without consulting a physician first.
     
    Does Sciatica Ever Require Surgery?
     
    In the event that following a conservative treatment regimen for several weeks and months has failed to provide meaningful relief, a patient may be advised to undergo an operation to decompress the sciatic nerve. Some patients may be candidates for minimally invasive procedures that are performed on an outpatient basis and can require less arduous rehabilitations than traditional open back surgeries. These endoscopic procedures are performed through a very small incision that is typically less than one inch long, and doesn't require the dissection of any muscles, tendons, or ligaments. Patients are generally able to return to daily activities within a matter of weeks, and many patients experience relief immediately following the procedures.
     
    If your physician has recommended surgical treatment for sciatica, ask him or her if you're a candidate for a minimally invasive procedure. Before consenting to any operation, it may be in your best interest to receive a second – or even third – opinion to ensure that surgery truly is your best treatment option.

    Spondylitis is a type of osteoarthritis that affects the sacroiliac joints in the pelvis and the facet joints in the spine. It's a chronic condition that has no cure, and can cause a variety of uncomfortable symptoms. Patients often experience pain and stiffness in the back first thing in the morning or after long periods of inactivity. The joints often become inflamed, which can lead to neural compression. In severe cases of spondylitis, the vertebrae (the bone structures that house and protect the spinal cord) can fuse, and the ligaments in the neck and back can calcify. Because severe spondylitis can require a very complex and highly individualized treatment plan, this article will only focus on less severe cases of the condition.
     
    Treatments for Spondylitis
     
    Many patients who receive a spondylitis diagnosis are able to manage their pain and discomfort through conservative, nonsurgical means. This often entails the use of one or more of the following treatments:
     
    NSAIDs – Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen and naproxen can reduce inflammation and relieve pain. NSAIDs aren't generally recommended for patients with some cardiovascular or gastrointestinal conditions.
     
    Analgesics – If a patient is unable to take NSAIDs, his or her physician may instead recommend an analgesic like acetaminophen.     Thermotherapy – Also commonly referred to as heat therapy, this treatment involves the application of a heating pad or other heat source to the neck or back. This sometimes helps reduce muscle tension, which commonly contributes to neural compression.
     
    Cryotherapy – Also known as cold therapy, cryotherapy involves the application of ice to a painful or inflamed area of the body. Ice can reduce inflammation and is also effective at numbing pain.
     
    Physical therapy (PT) – Strengthening and stretching specific muscles in the neck, back, and abdomen can sometimes help relieve the pressure being placed upon a compressed spinal nerve. Physical therapy also commonly entails transcutaneous electrical nerve stimulation (TENS), massage therapy, and various other treatment methods.
     
    Weight loss – Overweight patients can sometimes benefit from shedding excess pounds. The neck and back are already faced with supporting a great deal of body weight, and every surplus pound can place undue stress on the facet joints and other spinal components. Before embarking on any weight-loss journey, patients should consult their physicians first.
     
    Complementary and alternative medicine (CAM) – Though some CAM therapies aren't recognized by the mainstream medical community as legitimate treatments, many patients rely on them to relieve the symptoms of spondylitis and other spinal conditions. Popular CAM therapies include acupuncture, restorative yoga, chiropractic manipulation, and the use of herbal supplements.
     
    In the event that a patient's symptoms persist – or worsen – despite following a nonsurgical treatment regimen for several weeks or months, his or her physician may recommend surgery.

    Many patients who receive a slipped disc diagnosis feel confused, anxious, and maybe even depressed. They often wonder: "How did this happen?" "Why me?" "Will I be in pain for the rest of my life?" If similar thoughts are running through your mind, stop, take a deep breath, and consider this: Most patients who are diagnosed with a slipped disc will find relief from their pain and discomfort within a matter of weeks or months, and very few of these individuals will need to undergo surgery.
     
    Even still, you may have some questions about your spine condition. While you should consult with your physician or a spine specialist for answers to your specific questions, this article may provide you with some of the information you need to put your mind at ease.
     
    What is a Slipped Disc & What Causes it?
     
    A slipped disc (or, more accurately, a herniated disc) occurs when the inner content (the nucleus pulposus) of an intervertebral disc escapes through a tear in the disc's outer wall (the annulus fibrosus). More often than not, this occurs as a result of the natural aging process. Our discs act as shock absorbers and provide cushioning to the vertebrae, which are the bone structures through which the spinal cord passes. Over time, the burden of supporting the body's weight and the facilitating movement of the neck and back can take a toll on the intervertebral discs and they can begin to break down. As a disc's outer shell becomes weak and brittle, it becomes more prone to tearing and allowing its nucleus to escape.
     
    What are the Symptoms of a Slipped Disc?
     
    Believe it or not, many individuals have slipped discs without exhibiting any symptoms at all. These patients sometimes only receive a slipped disc diagnosis because an diagnostic MRI or CT scan for a totally separate issue has revealed a spinal disc herniation. However, those who do experience symptoms can feel radiating pain, numbness, tingling, and muscle weakness. These symptoms occur when the inner disc material seeps into the spinal canal and presses against a spinal nerve, nerve root, or even the spinal cord itself. The exact location of a patient's symptoms will vary depending on which region of the spine contains neural compression: When a nerve in the cervical spine (neck) is affected, symptoms can be experienced in the neck, upper back, shoulders, arms, and/or hands; neural compression in the lumbar spine (lower back) can cause symptoms in the lower back, hips, buttocks, legs, and/or feet.
     
    How is a Slipped Disc Treated?

    Spondylolisthesis is a spinal condition that occurs when one vertebra slips out of place and over the vertebra located directly beneath it. The condition is diagnosed in terms of grades, with Grade 1 indicating 0-25 percent slippage; Grade 2 representing 25-50 percent slippage; Grade 3 describing 50-75 percent slippage; Grade 4 indicating 75-100 percent slippage; and Grade 5 referring to complete vertebral slippage. Spondylolisthesis can occur as the result of a traumatic injury, but is most commonly caused by the natural aging process and the gradual deterioration of the intervertebral discs (which are responsible for supporting the vertebrae).
     
    Treating Spondylolisthesis
     
    Treatment for spondylolisthesis can vary from patient to patient and relies heavily on the extent to which the affected vertebra has slipped out of place. Generally speaking, patients who have been diagnosed with low-grade spondylolisthesis can find relief in the form of conservative, nonsurgical treatments. This could include a regimen of one or more of the following treatments:
     
    Thermotherapy/cryotherapy – More commonly known as heat/ice therapy, this treatment involves the applications of heat and ice. Applying heat to the affected area can help relieve muscle tension (a common contributor to back pain) and may aid in directing nutrient-rich blood to the area. Popular alternative therapies include the use of herbal supplements, acupuncture, chiropractic manipulation, massage therapy, and aromatherapy to name a few.
     
    Surgery for Spondylolisthesis
     
    Surgery for high-grade spondylolisthesis can be incredibly complex. Patients who have been diagnosed with high-grade spondylolisthesis should speak with their physicians about their unique surgical plans. For these reasons, this article will only focus on surgical treatments for low-grade spondylolisthesis.
     
    Some patients may receive little to no relief after utilizing conservative, nonsurgical treatment for several weeks or months and may be advised to undergo surgery to treat low-grade spondylolisthesis. Open back surgery sometimes involves the complete removal of an intervertebral disc and the fusion of the two adjacent vertebrae. While this renders the affected segment of the spine motionless, it can prevent vertebral slippage in the future. Recovery is generally long and arduous and some patients are faced with lengthy hospital stays. It should also be noted that patients who undergo open back surgery are at risk for developing failed back surgery syndrome (FBSS), which is the worsening or continuation of symptoms following an open back operation.
     
    Before consenting to any operation, patients should strongly consider receiving a second – or even a third – opinion.

    A sciatica diagnosis doesn't equate to a lifetime of pain and misery. In fact, most patients who are diagnosed with sciatica will find relief from their pain and discomfort in the form of conservative, nonsurgical treatments. In some cases, however, a patient will require surgery to decompress the sciatic nerve, but only a very small minority of patients will fit into this category.
     
    Nonsurgical Treatments for Sciatica
     
    Most patients who receive a sciatica diagnosis will be advised to utilize one or more of the following conservative, nonsurgical treatments:
     
    Medication – Over-the-counter, nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen interfere with the body's inflammatory response in an effort to relieve pain. Some patients – especially those who suffer from certain cardiovascular or gastrointestinal conditions – may be unable to take NSAIDs and might be advised to take an over-the-counter analgesic like acetaminophen instead. If over-the-counter medications prove ineffective, a physician or spine specialist may prescribe a narcotic pain reliever and/or muscle relaxant.
     
    Hot/cold therapy – Applying heat to the lower back can ease muscle tension (which may be contributing to neural compression) and direct nutrient-rich blood to the area. Ice, on the other hand, can relieve inflammation and numb away pain. Some patients will be advised to alternate between heat and ice while others may be directed to use one or the other exclusively.
     
    Physical therapy (PT) – Working with a physical therapist to strengthen and stretch specific muscles in the lower back could prove beneficial to patients who have received a sciatica diagnosis. PT may involve other techniques as well, including transcutaneous electrical nerve stimulation (TENS), massage therapy, and traction, among other treatments.
     
    Losing weight and other lifestyle changes – Carrying excess bodyweight can unnecessarily contribute to the compression of the sciatic nerve and, in some cases, it could actually be the root of some patients' spinal issues. Safely losing weight through diet and exercise can provide many patients with the relief they need. Other lifestyle changes – including quitting smoking, reducing alcohol consumption, and becoming more physically active – can also sometimes reduce or relieve the symptoms of sciatica.
     
    Surgery for Sciatica
     
    After following a regimen of conservative treatments for several weeks or months, some patients may still experience the painful and uncomfortable symptoms of sciatica. These patients might be advised to weigh the pros and cons of undergoing surgery. While surgery for sciatica once required a long hospital stay and an arduous recovery, it can now be performed on an outpatient basis and may only require a few weeks of rehabilitation. These minimally invasive sciatica treatments don't involve the complete removal or fusion of any spinal components, nor do they require the dissection of any musculature. However, not all patients who have received a sciatica diagnosis are eligible for a minimally invasive procedure.
     
    If you've been advised to undergo surgery for sciatica, ask your physician if you're a candidate for a minimally invasive procedure instead. Before consenting to any operation, you may want to pursue a second or third opinion to make sure surgery really is your best treatment option.

    If you're considering a career as an orthopedist, you'll need to do a lot of research to decide if the job truly is right for you. One of the things that you'll want to consider is the types of conditions that you'll be required to treat. An orthopedist can treat musculoskeletal conditions that affect anything between the top of the head and the bottom of the feet, including the spine. Here are some of the common spine conditions that you may be required to treat as an orthopedist:
     
    Degenerative disc disease – This condition affects the intervertebral discs, which are small pads located between adjacent, articulating vertebrae (the bone structures through which the spinal cord passes). The intervertebral discs act as shock absorbers and provide cushioning to the vertebrae. Over time, these responsibilities can begin to take their toll on the discs, causing their outer shells (annuli fibrosi) to harden and their jellylike centers (nuclei pulposi) to dehydrate. These changes make the discs prone to herniating and bulging, which can cause significant pain. A herniated disc occurs when the disc's annulus fibrosus tears, allowing the nucleus pulposus to escape and enter the spinal canal. A bulging disc occurs when the annulus fibrosus weakens to the point that it can no longer maintain the disc's shape, allowing the nucleus pulposus to shift to one side. Treatment for degenerative disc disease can include conservative treatments – such as the use of pain medication or the application of heat and ice – and/or surgical treatments to decompress the affected spinal nerves.
     
    Facet syndrome – Also sometimes called facet disease, facet syndrome is a type of arthritis that affects the joints in the neck and back (the facet joints) that connect adjacent, articulating vertebrae. The facet joints facilitate every bend and twist the neck and back make which, over time, can add up to a significant amount of movement. It's no wonder that the joints' cartilaginous lining can eventually wear away, leaving them with little to no lubrication. The joints can become stiff and inflamed and patients may find it difficult to move first thing in the morning and after long periods of rest. In an effort to mitigate painful articulation of the facet joints, the body may produce an osteophyte (bone spur). In theory, the osteophyte serves to stabilize the joint and prevent further wear. In reality, the osteophyte can do more harm than good by invading the spinal canal and causing neural compression. As an orthopedist, you may be tasked with removing the osteophyte and/or using a bone graft and hardware to fuse the surrounding vertebrae.
     
    Sciatica – This condition occurs when the sciatic nerve (the body's largest nerve) becomes compressed. The nerve originates in the lower back and travels down the leg and into the foot. When it's compressed by a misaligned, inflamed, or otherwise damaged component of the spine, the patient can experience a variety of uncomfortable symptoms throughout the lower body. This can include pain, numbness, tingling, and muscle weakness in the lower back, hip, buttock, thigh, and/or foot. Surgery may be required to decompress the nerve and alleviate symptoms.
