Nurses are often the most memorable part of a medical experience for a patient because nurses are the men and women that seem to always been there to allay fears and soothe worries. They are often available to answer questions when doctors are not, and some nurses can even diagnose conditions, prescribe medications, and do almost as much as their medical doctor counterparts.
There are many different types of nurses and it can be overwhelming to think about all of the different specialties, especially if you're considering a nursing career. Choosing what type of nurse you want to become can be difficult, but you can begin narrowing down your options by reviewing some of the categories into which nursing professions loosely fall: level of education, medical specialty, population, and by facility/medical department.
Level of Education
Nurses are often distinguished by the level of education they've received, the type of degree they hold, or the certifications they have earned. Licensed practical nurses (LPNs) have completed the least amount of education, which is typically one year of coursework following high school graduation. Conversely, to become a registered nurse (RN), you must complete a diploma, associate, or bachelor's degree program in nursing, in addition to passing the RN certification exam.
Advanced practice registered nurses (APRNs) are RNs who have continued their education journey beyond undergraduate school to earn a master's degree. In most cases, APRNs will take the opportunity to study a specific medical specialty. APRNs include:
• Clinical registered nurse anesthetists (CRNAs) – can administer anesthesia and monitor patients who are anesthetized
• Clinical nurse specialists (CNSs) – are typically trained in community health, psychiatric, or cardiac fields
• Nurse practitioners (NPs) – can diagnose and treat a wide range of physical and mental conditions, within their scope of practice; can also prescribe medications
Medical Specialty
You may decide to become a nurse based on a medical specialty that you have an interest in. For example, you may be interested in obstetrics and gynecology and decide to pursue a path as an OB/GYN nurse. Other nursing specialties include surgery, occupational health, oncologic, emergency and trauma, psychiatric, and midwifery.
Population
Aside from medical specialty, you may be more interested in helping a specific population of people. If you have a particular affinity for helping the elderly, you could work toward becoming a geriatric nurse. Or, if working with children is more your style, perhaps specializing in children's health and following a pediatric nursing path is right for you.
Facility/Medical Department
Another category that you could choose to explore includes nursing professions by a specific type of facility or medical department. For example, you might decide to pursue a career as a hospital nurse, a school nurse, or a hospice nurse. Working in a hospital could provide you with many other types of nursing tracks. If you like the adrenaline rush of emergent and traumatic situations, working in the emergency room might be for you. Or, if you prefer helping out during detail-oriented surgical procedures in the operating room, you may choose to train as surgical nurse.
Ultimately, a combination of your experience, education, and certification will determine which nursing career path you'll embark upon. For this reason, it's important to thoroughly research the different types of nurses and select from a few that suit your personality and skill set the best. Additionally, prior to spending significant amounts of time and money on a nursing degree and license, talk to friends or family members who may be nurses and find out what they believe to be the pros and cons of their chosen field.

According to the National Institute of Neurological Disorders and Stroke, back pain is one of the leading neurological ailments that affect people, second only to headaches. Back pain is also a common reason for missed work days, and billions of dollars are spent annually on treating spine-related discomfort.
Back pain and other symptoms can vary in severity, but physician-recommended treatment usually begins conservatively, or nonsurgically. Some commonly suggested treatments include pain medication, hot/cold therapy, and non-steroidal anti-inflammatory drugs (NSAIDs). Physical therapy is another common treatment method that focuses on restoring physical function and helps make performing everyday tasks easier for patients hindered by back pain. Physical therapy typically includes a variety of modalities, such as transcutaneous electrical nerve stimulation (TENS), ultrasound therapy, and massage, but a large part of improving body mechanics involves exercises that stretch and strengthen abdominal and lower back (core) muscles as well as leg muscles.
Types of Exercises and Stretches
Before you begin any exercise regimen, first consult with your physical therapist to ensure that you know how to perform each exercise correctly, which can help limit additional injury.
The following are some back exercises that your therapist may have you perform:
• Pelvic tilt – Lie on your back with feet flat on the floor and knees bent. Take a few deep breaths and when ready, tighten your abdominal muscles as you exhale and press the small of your back into the floor. Your pelvis should roll slightly upward. Hold this position for a few seconds and then return to the starting position.
• Prone back extension – Begin lying on your stomach. Prop your upper body up on your elbows, extending your back. Then slowly straighten your elbows to deepen the spinal extension until you feel a stretch and hold for a few seconds. Lower your upper body.
• Cat stretch – Get down on your hands and knees and begin by holding your back parallel to the floor. While engaging your abdominal muscles and pulling them toward your lower back, gently arch your back toward the ceiling until you feel a stretch and hold. Return to the starting position.
• Straight leg raises – Lie on your back with one knee bent and one leg straight. Tighten your abdominals and slowly lift the straightened leg straight up several inches from the floor. Hold this position for several seconds, and then lower your leg. Perform the exercise with your other leg.
• Knee-to-chest stretch – Lie on your back with your feet on the floor and knees bent. Grasp one thigh behind the knee and gently pull toward your chest until you feel a stretch in the lower back. Hold this position for several seconds and release. Perform the same stretch with the other leg.
Without hurting yourself, you should try to repeat each of these exercises several times. For the best results, perform the exercises several times per day, or as directed by your physician.
Exercises that involve use of an exercise or physio ball may also help you mitigate and prevent back pain episodes. Performing exercises on an exercise ball introduces a level of instability that traditional floor exercises can't offer. Your body will respond automatically to this instability, and will engage a variety of core muscles to keep your body or legs balanced on the ball. Additionally, physio ball exercises offer a controlled method of moving vertebrae and gently "pumping" intervertebral discs up and down. Pumping discs increases nutrient- and oxygen-rich blood flow to areas around the disc and helps flush cellular wastes and toxins. Additionally, disc pumping can also hydrate discs by causing water to flow in and out of them.

As dank storage spaces, basements often lack proper lighting. This might not seem like an issue when you don't spend very much time in the room, but if you're planning to renovate your basement, poor lighting can be a big problem. In addition to keeping you safe by lighting your way, the right fixtures can add some ambiance to your basement and banish the darkness normally associated with rooms that are underground. Of course, choosing your lighting solution will depend upon your planned use for your basement, but here are some basic ideas.
First, you should make the most of any natural light you might have access to, including any aboveground windows. Windows are excellent for letting light into the room, and you can reflect more of it throughout your basement by painting the walls a lighter color.
If you like the look of a window with light streaming through it, but you don't have the benefit of a window, you can create a faux window. Simply frame out an area for a window, trim it, paint the wall behind it white, and install some bright daylight-spectrum bulbs. Finish the window with blinds or sheer curtains so the illusion will be complete.
One of the most common choices for basement lighting fixtures is recessed can lights. If your basement has finished ceilings, you may want to consider cutting holes in the drywall or tiles, and sinking these canisters into them. These lights are easy to install and provide ample lighting for virtually any activity. They're even available in a variety of sizes, so you can choose whether you'd rather light your basement with a few larger recessed lights or many smaller ones. In case you're worried that having hot lights so close to your ceiling will result in a fire, remember that many of the newer recessed lighting fixtures have a mechanism to automatically switch the lights off if they begin to overheat.
Track lighting is also popular because it consists of multiple adjustable lights that hang from a track that runs along the ceiling. Like recessed lighting, track lights provide a wide swath of light, but they can also be moved independently to cast light from a variety of angles.
If you need supplementary lights to better illuminate small areas within the basement, there are always pendant lights. These lights provide focused, brilliant light that might be necessary over pool tables, dining areas, wet bars, home offices, and anywhere else where you really need to see what you're doing. There are many pendant fixtures on the market, so you'll be able to find an appropriate solution for your basement, no matter what style you like.
There are many other types of lighting on the market today, so feel free to explore all of your options. Just remember that it may take a combination of several types of lights to properly illuminate your basement, and you'll need to take the time to visualize how all of your fixtures will work together.

In the past, basements were almost always musty spaces thanks to the fact that they received very little (if any) natural light. Because they're underground, basements are also often damp, humid spaces. Unfortunately, mold thrives in dark, damp spaces, which is exactly why many basements are infested with these smelly, potentially dangerous fungi. Some of the new finishing methods are made from breathable, inorganic materials that don't support the growth of mold or mildew. You should keep in mind, though, that these methods can only prevent mold from growing and won't resolve an existing mold issues.
Depending on where you live, you may also want to make sure that your basement finishing method of choice has built-in insulation. Even when outside temperatures are relatively comfortable, the basement can be a downright chilly space. If you live in a colder climate and plan to use your basement year round, then you'll want to choose a method that provides ample insulation. The last thing you'll want to do is invest in a basement remodel that can only be used a few months out of the year.
You should also make sure that you choose a basement remodeling method that is compatible with how you plan to use your basement once it's finished. For instance, if you're transforming the space into a home gym, then protection against mold and mildew is especially important (think about all the extra moisture that will be introduced into the space in the form of sweat… ick!). If you're creating a spare bedroom, mother-in-law suite, private office, or other type of space that requires year-round use, then you'll definitely need to think about insulation.

Make sure the VPS offers a wide range of Linux distributions, or at least the distribution that you prefer. Some like CentOS for its stability, others like Ubuntu for its ease of use. If you're not sure which distro you prefer, select a VPS with a lot of options.

Is it possible to fit a visit to London in a single day? There's an public underground transportation system that operates relatively quickly. Almost all underground station exits have signs pointing to important landmarks, helping tourists find their next location. London is a terrific city to squeeze into a single day trip. Here's a short guide on how to do that. I've also included directions on how to get from one location to the next using their public transportation, lovingly called the tube.
Begin with an educational experience at the British Museum
Since you only have a limited amount of time, I suggest you go to the British Museum's website and research which artifacts you want to see before your trip beings. The museum doors open by 10AM on most days, which is when you should arrive, although on some days they open earlier. The British Museum is completely free to visit. Since their opening in 1753, the museum has managed to collect over 8 million items from multiple cultures. The British Museum was founded in 1753. Make sure to arrive early in the morning to avoid the large crowds. Popular artifacts are the Rosetta Stone, the Elgin Marbles, and the Beowulf manuscript.
After exiting the museum, go west along Great Russell Street and make a left on Tottenham Court Road. Use the Tottenham Court Road tube station.
See what royalty is like at Buckingham Palace
Get off at St Jame's Park station and walk up the stairs. Walk west on Petty France Street and make a right onto Buckingham Gate Road, you'll run into a palace!
Buckingham Palace open for tourists to go inside. In Buckingham Palace, you'll find the Queen's Gallery which features art pieces from the highly esteemed Royal Collection. Outside the palace, you might be able to catch the Changing of the Guard ceremony, in which two guards mount and dismount horses. It occurs once a day and lasts about 45 minutes. Buckingham Palace currently serves as the royal family's residence while Windsor Castle, outside of London, serves as their vacation home.
Enjoy the scenery while walking along the River Thames
From Buckingham Palace, go back to the St Jame's Park underground station, but don't get onto the tube. Instead, walk straight past it and you'll find yourself on Victoria Street. Here, you'll eventually walk to the Westminster Abbey, the church where kings and queens are crowned and married. Built in the 970s, Westminster Abbey has served as the royalty's top venue for coronations and marriage ceremonies to this day. It's also the final resting place for several important historic figures.
From Westminster Abbey, head north along Ablingdon Street, then make a right onto Bridge Street. You'll find a giant clock tower that will catch your eye.
The official name of Big Ben is the Elizabeth Tower. The secondary bells chime every quarter hour, while the main bell rings every hour. Big Ben was constructed in 1858 to stand 315 feet high. Big Ben is actually the nickname of the main bell - which weights over 13 tons. The tower is a cultural icon of London.
Continue walking down Bridge Street until it turns into the Westminster Bridge. Walk along the bridge to cross over the River Thames. Enjoy the view while you're there! Humans dating back to 2700 BC have enjoyed the water from this river, when civilization first settled here. The River Thames is the longest river in this country and the second longest in the United Kingdom.
At the end of the bridge, turn left onto Belvedere Road. Keep walking and you'll run into the beautiful Jubilee Gardens. If you look straight ahead, you'll see a giant ferris wheel. That's the London Eye. The London Eye is the tallest ferris wheel in Europe and is 443 feet high at its peak. A ride on the ferris wheel lasts about 30 minutes and has fantastic views of central London and the River Thames. Consider going for a ride if you have time. Chance are, if you followed this guide, you'll be here around the evening. Wait a bit longer for the night sky to settle in and the ferris wheel will be lit up with blue lights!

A new must-read for expecting moms is Bridget Swimney's recent fourth edition of "Eating Expectantly." Although the primary focus of the book is pregnancy nutrition, it also offers healthy lifestyle advice for the whole season of pregnancy, including pre-conception, pregnancy, postpartum and breastfeeding. Lifestyle emphasis goes beyond diet to living in a cleaner, greener and safer environment for you and your baby. This includes discussion on exposure to cosmetic and household chemicals during pregnancy.
As to the subject of pre-natal vitamins and supplement, much new research has been published lately on the importance of Vitamin D during pregnancy. In the past stress was given primarily to Folic Acid, which is still an important supplement. However, Vitamin D is an underestimated important supplement that many pregnant and breastfeeding women to not get enough of. Inadequate Vitamin D has been linked to gestational diabetes, pre-eclampsia and decreased bone mass in newborns. New research now suggests that most women start their pregnancies with deficient Vitamin D levels, so the recommended levels of 600 IUs in insufficient. Now researchers are recommending pregnant women supplementing at a minimum of 1000 IU. Now vitamin levels can be tested to find out what dose is best for a person's specific needs.
Another important diet component during pregnancy is iodine which controls metabolism and is important in brain development. It's important for pregnant women to get an adequate supply of iodine in their diet to protect from birth defects and impaired cognitive function. Most people get most of their sodium from sodium based preservatives and adding salt to their food. However, not all salt contains iodine, including sea salt. A good way to get iodine in your diet is to buy iodized salt and eat foods already rich in iodine such as milk, potatoes and cod.
Choline is another nutrient that has not been stressed much in the past for pregnancy but is important for the developing brain. Recent studies have shown mothers who did not get enough choline in their diet had higher occurrences of neural tube defects in early pregnancy. Good food sources for choline include eggs, cod, lean beef and Brussels sprouts.
Swimney also cautions against substituting artificial sweeteners for women with gestational diabetes or just trying to watch their sugar intake. Research from Denmark showed an increase in pre-term deliveries for pregnant women who consumed more than one beverage daily with aspartame sweetener. While the study is not conclusive, it does give pause to consuming too much of these sweeteners during pregnancy or breastfeeding.
As far as healthy pleasure foods, Swimney gives a thumbs up to dark chocolate and natural cocoa. New research shows that these chocolates improve blood vessel function which is linked to improved cardiovascular health. Regular chocolate consumption is also shown to reduce pre-eclampsia and hypertension. That's a good reason to indulge in chocolate milk, hot cocoa or a few chocolate chip cookies!
Swimney gives more details on these health recommendations and many more in her fourth edition, "Eatting Expectantly." It is definitely worth checking out to assure you are doing everything you can to ensure a safe and healthy pregnancy.

The Grand Canyon's magnificent South Rim is in northern Arizona, close to the Utah border. It's often referred to as the "official" Grand Canyon and it's the part of the National Park where you'll see the most spectacular vistas. The canyon covers a million acres - way too much to see if you stay on the ground. That's why seeing the Grand Canyon by airplane is such a popular option – in less than 60 minutes you'll see places that not many travelers ever do!
The South Rim is the perfect day trip from Flagstaff, Sedona, Phoenix, Scottsdale and other Arizona cities.
A lot of folks ask if they should take one of the plane tours that start in one of those cities. It's actually cheaper and more fun to rent a car or SUV, drive to Tusayan, Arizona, and then fly from there. Tusayan is just outside the Park's main gates and has all the services you'd need, including restaurants, hotels, fuel and stores. At about a 1 ¾ hour-drive, Flagstaff is the closest to Tusayan of those four Arizona cities. The drive from Phoenix is the longest, at about 4 hours. If you'll be flying to Tusayan, flights from Phoenix are probably the cheapest.
Plenty of travelers coming from Vegas go to the South Rim, too. Because the one-way trip is about 275 miles, I always suggest flying instead of driving or taking a bus. It's a direct flight and it takes less than 60 minutes. Getting there on the ground would take 6 or so hours. That's a total of 12 hours just to get to the South Rim and back!
Vegas plane tours are a terrific value because a 2 ½-hour bus tour inside the Park is bundled into the package. During your tour you'll see a number of South Rim highlights, including:

Yaki Point
Yavapai Observation Station
Grand Canyon Village
An upgraded version of this tour adds an exciting 30-minute chopper flight from the South Rim to the North Rim and back. If you want a way to surprise Mom on her big day, then you should take her on an airplane tour of the Grand Canyon. Whether you pick a landing tour or an air-only version, you're all sure to have a great time!
Airplane tours fly out of Grand Canyon National Park Airport (GCA) at Tusayan, Arizona and from airfields in the Las Vegas metro area. Folks coming from AZ will fly to the South Rim, but Vegas-based travelers can choose from flights to either the South or the West Rim.
Can you think of anything more thrilling for Mom's big day than whisking her away from Sin City for the fun and excitement of a Grand Canyon airplane tour? The first thing you have to do is decide which one of the rims you want to go to.
If your mother loves natural beauty, take her to the South Rim ? a lot of people consider it the real Grand Canyon. If your Mom is the adventurous sort and loves a few thrills, then you should take her to the West Rim.
West Rim Flights
When you fly to the West Rim from Vegas, you also get to see the historic Hoover Dam and beautiful Lake Mead. Most air-only tours fly over all the main West Rim highlights (including the Grand Canyon Skywalk) before returning home. There are also landing tours available and these are a lot of fun so you should consider taking one if you can. Some terrific side excursions are available.
For instance, you can treat your Mom to a landing tour that includes a VIP pass to the Skywalk or Glass Bridge. There she will walk 70 feet past the edge of the canyon wall, stand on a glass sidewalk and look into the depths 4000 feet below her. It is a once in a lifetime experience!
She would probably also enjoy a chopper flight to the floor of the Grand Canyon. The West Rim is the only place you can do this (South Rim regulations prohibit flying below the level of the rim), and it's a truly unique experience. It's a spectacular flight that's made even better by the fact that the package also includes a smooth-water rafting trip down the Colorado River!
South Rim Flights
Airplane tours from Las Vegas leave two or three times daily to go to the South Rim. You will get there in about an hour. Included in your airplane tour to the South Rim is a motor coach tour of the National Park that is about two hours long, plus you can add on a helicopter tour of the region too.
The 50-minute helicopter flight is a good option because it lets you see the East, South, and North Rims of the canyon in a single tour. In fact, the tour takes you over about ? of the entire National Park.
Something else you can add on to your South Rim tour is a Jeep ride through the Antelope Canyon where you Mom can see some amazing rock formations. The trip ends with a real flourish ? a smooth-water rafting tour down the Colorado! What could be better?
Wrapping Up
I've given you several ideas for Grand Canyon tours this Mother's Day (Sunday, May 12). The airplane tours fill up fast though so don't wait to buy your tickets. Buy your tickets online at the tour operator's website and you will save enough money to treat Mom to dinner when you get back to your hotel!

A good percentage of Californians get their health insurance through companies, the government (State, Local, and Federal), Military, or State programs (Medi-Cal, Medicare, etc). That still leaves a large number of people that need to find and purchase California personal health insurance. Purchasing personal health insurance is usually both an expensive and confusing endeavor so let's try to simplify it and offer guidance on how the find the most affordable personal health insurance in California. Before we jump into that process, let's discuss who is in the market for personal health insurance and what their options are.
Who buys personal health insurance?
Basically, the purchaser of personal health coverage is anyone who doesn't have coverage somewhere else. A fair number of these shoppers are self-employed or small business owners (think mom and pop type businesses). This includes 1099 contract workers who are essentially self-employed. You also have employees of companies that do not offer group health insurance. This dynamic might change a little with health reform but not much since the law doesn't apply to small businesses in California (under 50 equivalent employees) and the majority of employees work for small companies. You also have dependents of employees who only get group health coverage for themselves (and not the dependents). Add in students, artists, kids, and that wraps up who generally finds themselves looking for personal California health insurance. So what does the market look like and what are the pro's/con's of personal health coverage?
The personal health insurance market pre and post health reform
The personal health market is very different from group in California in that it is currently medically underwritten. You can be declined due to health status/history until Jan 1st 2014. You can find more detail on the personal market at our Understand Individual health insurance page. Since personal health insurance is medically underwritten, the rates tend to be much less than group health for comparable coverage. The benefits also tend not to be as rich as company insurance since people are paying for their own coverage and that tends to push down benefits over time especially with the history of rate increases. Starting Jan 1st, personal health insurance will be guaranteed issue (regardless of health) like group health currently is. It will also be much more expensive than current available plans. One note...self employed people (including S-Corp officers/owners) may be able to deduct 100% of the their personal health insurance premium. This is a huge advantage that the non-self employed do not have.
Current plans on the personal health insurance market in California
We've written extensively about the best health plans available but here's the important pieces. HMO's have priced themselves out of the market. We're really only insuring for a short period of time since all the current personal health plans on the market will go away Jan 1st (or on your anniversary month in 2014). It's hard to justify going up the scale of benefits for personal coverage. The mid to low range plans generally make the most sense. Catastrophic or Major Medical personal plans are by far the most popular these days. You can run quotes and apply for California personal coverage online and instantly through our quote page. Rates are primarily driven by age and area although health status can affect the core rate available. Contracts are month to month and can be cancelled for any reason. Once approved, your individual rate cannot be increased due to your specific health situation going forward. There are no lifetime or annual caps. It's the beginning of May, 2013 and we're getting closer by the day to the full ramp up to Health Reform and "H Day" of Jan 1st 2014. Okay, so the H Day name hasn't caught on yet but the dates are important none the less. The next big piece of information to drop will come smack in the middle of summer and it's the real question mark to this whole roll out. The carriers have scheduled to release the new health plan benefit summaries and rates at that time and everything hinges on that information. Let's look at what to expect and how to best prepare as California individuals, families, and companies.
Road hits pavement summer of 2013
We've been warning of news related to expected rates coming Jan 1st for most Californians but until we see the actual numbers, it's all just speculation (even if educated speculation). This is what we expect based on multiple accounts and sources. For individuals and families, the rates and benefits might be quite different. The expected rate difference (we won't call it an increase since the benefits are different) will be likely be 30-60% depending on a few factors. All rates will go up but not equally. Younger adults are likely to absorb the greatest increase due to the 3-1 age band compression which we've discussed is a separate articles. We can expect "younger" to range up to about 40-45 years old. Older adults not eligible for Medicare (age 45-64) will likely see the lower range of this rate differences (say 30%) as they benefit from the rate compression. Some Californians (up to 400% of Federal Poverty Level) may be eligible for health premium subsidies which will offset the premiums significantly.
What about plan benefits?
Keep in mind that we're comparing what people are paying now to what they will like pay then. California has already released the essential plan benefits and although the carriers may offer slightly different versions, they can't differ to much and still hit the actuarial level. The Platinum and Gold plans are so rich in benefits that we don't see many people choosing these plans as the costs will be astronomical. The California Silver health plan is still very rich compared to today's health plans so we also don't expect as much activity here other than the Exchange (people who qualify for a subsidy). For people that don't qualify for a subsidy, this basically leaves the Bronze health plan which is where we see 80% of the market going due to cost. It just won't make sense to go up the scale in terms of plan benefits when considering cost difference. So we have Silver in the Exchange and Bronze off the Exchange (individuals under age 30 will also have a catastrophic health plan option). You can see the core benefits already here.
What about California group health insurance?
Group health plans will likely not see such an extreme premium difference as the individual/family market. The California group health market has already incorporated many of the mandates which are new to the Individual market such as maternity, mental health parity (California State law), etc. We expect rates to increase about 15% in California although the plan choice may affect this since many groups have opted for higher deductible options which may not directly correlate with the 4 plans available as part of Health Reform.
As soon as we get the new rates, we'll make sure to notify our clients and Facebook followers so they can see what to expect. In the meantime, keep your fingers crossed!

We're always a little leery of anyone stating that something is the "best" especially with a complicated product like California health insurance. That being said, 80-90% of the dozens of health plans on the market do not make financial sense. We can safely remove them from our candidates for the "best California health insurance plan". So why is this case and which plans are then left over to consider. Now those are questions we can answer after 20+ years in the health insurance market and 1000's of clients (and health plans). We've seen the best and the worst (always changing by the way) so let's share some insight on how to find the best California health plans and actually list out what they are on the current market.
So how do we define best California health insurance plan?
It's simple and it is consistently what everyone asks for when we speak with them. "The best coverage for the lowest rate". That is literally the answer we get and it's always with a hint of sarcasm as in "duhhhh". Since most people don't deal with health insurance daily, the jump from that simple and concise statement to an actual plan selection leaves people with confusion and doubt. Let's boil down that high level statement to something we can actually use to pick the best health insurance plan in California.
Total potential out of pocket versus total guaranteed out of pocket
This is really the measure we need to look at in order to find the best coverage. How much in guaranteed cost (your annual premium paid) will we pay versus how much is potential total cost (expected medical out of pocket if we get really large bills plus a smaller consideration on what we'll pay for more common smaller bills). That's it! That defines the best health plan choice for you. We want to lower our monthly or annual premium to a point where it doesn't make sense to go lower in case we get really big bills. Let's take an example.
Example of our ideal best health insurance plans
Let's say we're considering a $2000 deductible plan with a $3500 max out of pocket ($5500 real exposure for the big bill). It's $300/monthly. Now let's say we're comparing it with a $3500 deductible plan with a $3500 max out of pocket. True exposure for the big bill is $7K and the premium is $150. The latter exposes us to $2K more if something big happens but we save $1800 annually in premium whether we use the plan or not. That may sound like an extreme example but it isn't on today's market. In that case, we'll save a guaranteed $1800 versus a potential of $2K. That's a smart bet and one of those plans is definitely "better" all other things being equal. So, let's provide you a short cut based on our daily experience. What's the best health plan today for Californians?
Today best California health insurance plan
Individual and Family:
On the individual and family market leading up to health reform changes (we'll address separately when we get the rates), these are the best health plans based on our experience with increasing benefit and cost going down:
More catastrophic coverage:
Health Net Advantage $3500 or $6500 (we like the $3500 better)
Anthem Blue Cross Clear protection (lab, x-ray goes to max out of pocket but pricing is great and carrier network/history is solid)
Mid-level benefits/costs:
Anthem Smart Sense $3500 or Shield Wise $3500 whichever is cheaper for your age/zip
Blue Shield $4000 Savings for low max out of pocket and HSA compatibility
That's it! Yes, 4-5 plans. You'll be hard pressed to justify going up or down the scale from these plans and the catastrophic health plan options are generally the lowest priced plans on the market.. We have had two decade of health insurance experience and the health reform changes still require a great deal of study to understand. We can imagine how it must feel for the average American who is trying to see how it applies to them. There's a host of new terms with critical impact on how much you will pay for health insurance. One of these is Health Exchange. Let's understand what exactly the Health Exchange is and how it affects your options for a health subsidy.
First, what is the Health Exchange?
The Health Exchange is a marketplace to buy specific health plans starting Jan 1st, 2014. These are referred to as Exchange Plans and they are also called the Metallic health plans. There will be a Bronze, Silver, Gold, and Platinum with a Catastrophic option for younger people. There will also be other plans outside of the Exchange and we'll discuss how these might be different. You will be able to quote both Exchange plans and non-Exchange plans through our Health Subsidy Quote page once they're available (Oct 1st, 2013). This ideal since the Exchange will only be able to quote plans inside it which may or may not be the best value for you. What will dictate this?
Subsidies will form the dividing line for Exchange Plans
If you make less than 400% of poverty (the 400% Rule), it will be hard to beat the eligible Exchange plans since you will likely qualify for a health subsidy which is after all, the whole point of this entire website. It's potentially a lot of money at state...$1000's of dollars per year for most people. Your Quoting page will automatically determine if you qualify for a health subsidy and towards which plan at no cost to you. Only the qualified Exchange plans (should be the Silver health plan) will be eligible for a subsidy so that will narrow your decision making down considerably since the the total premium will probably be subsidized by 50-80% depending on your income level. This will supersede any other plans/rate differences that might occur in most situations. What if you don't make under 400% of the Federal Poverty Level? Do the Exchange Plans still make sense?
Differences between Exchange and Non-Exchange plans
The first consideration will be whether you have a Grandfathered plan. We've discussed this consideration in depth at our Grandfathered plan versus Exchange plan page. It's important to understand how that works. If you have a non-Grandfathered health plan, there may be advantages to non-Exchange health plans. We're starting to see a blueprint for non-Exchange health plans with much larger doctor/hospital networks than Exchange plans. The core benefits for non-Exchange plans have to match the Exchange plans but the in-network doctor networks may be half the size of current networks inside the Exchange. This could be a big deal. The costs will likely be the same but if you want the better doctors/hospitals, non-Exchange plans might be the way to go. healthsubsidy.net is mainly about getting the most health subsidy that you are eligible for and we're serious in that aim. After all, it could mean 1000's to 10's of thousands of dollars per year to you and your family. Those are serious numbers and we felt it warranted an in-depth discussion on how to maximize it. There's another important change resulting from the Health Reform law that we need to discuss...Guaranteed Issue health plans.
A quick definition of Guaranteed Issue health insurance
This is a major change in the health insurance market for individual and family health insurance. A major issue in purchasing health insurance on the individual market (and depending on the State, the group health market) has been health qualification. In most markets, an insurance carrier could decline coverage or increase the premium charged based on the health status and history of the application. Depending on your State and the carrier, roughly 50-80% of the applications might be declined or rated up. This has been a big problem in the health insurance market despite various high risk pools and the temporary PCIP plan as part of the Health Reform bill. It often left people with no or very bad options. That's going away Jan 1st, 2014.
Effective Jan 1st, 2014, health history and status will not impact qualification
It's hard to say whether Guaranteed Issue on the individual/family market or the Health Subsidy is the biggest change coming our way but they're both up there. Here's the landscape for the Guaranteed Issue changes.
An eligible person cannot be declined coverage due to health status or history. Essentially, the whole issue of pre-existing conditions go away. Health history goes a little deeper and speaks to past health issues which previously would affect a person's ability to qualify such as certain types and stages of cancer in one's history. An insurer can also not increase a person's insurance premium based on their health status/history or apply waiting periods or waivers to pre-existing conditions. A person's premiums can also not be increases later on if health changes. Keep in mind that rates can increase by other criteria such as age bands, area, and based on claims experience for large groups (such as by State). Rates just can't be increased based on your individual claims experience for you alone.
Windows for Guaranteed Issue Enrollment
Depending on each State's interpretation of the rules, there will likely be open enrollment windows (such as October 1st through Dec 31st) and certain qualifying events during the year (such as losing group health insurance or moving States). If you do not enroll during these windows, you will likely have to wait till the next window. This restriction plus a penalty for not purchasing health insurance will be the "stick" to motivate people to enroll while the health subsidy will be the "carrot". Guaranteed Issue is applicable to both plans in and out of the Exchange.

The Health Subsidy which we describe in detail offsets the cost of health insurance premium (the amount you pay each month to keep the policy in force. Benefit Subsidies is something entirely different but equally important for those that qualify. Let's understand how this works!
What exactly is the Benefit Subsidy?
Benefit Subsidies is exactly that...where health subsidies will offset your premium cost, benefit subsidies will offset your actual out of pocket for medical care itself. The ACA law stipulated that most people making under 400% of poverty would qualify for premium subsidies but they realized that even with insurance premium offsets, many people in the lowest margins (but higher than 150% of FPL who qualify for free Medicaid) would still have difficulties paying their out of pocket medical expenses. Even with a richer plan, the out of pocket maximum of $1000 (depending on plan, State, etc) would be hard for someone making 150% of poverty (roughly $16K annually). To compensate for this, they also devised benefit subsidies for people making between 150% and 250% of the Federal Poverty Level...the lower segment of range for those that qualify for the health subsidy. Let's see how this works.
What are the different levels of Benefit Subsidy?
First, there may be different interpretations of how to actually implement this benefit subsidy. For example in California, they just announced that there would be multiple Silver health plans based on income (150%-200%, 200-250%, and standard Silver). The 200-250% plan has richer benefits than the Standard Silver (say for someone making between 250% and 400%). The 150%-200% plan will have richer benefits still than the 200%-250% plan. The difference in benefits are significant at each level so this benefit subsidy means real money for those most in need. Other States and the Federal Exchange might have different takes as well although California is usually out front on these benefit changes. There has been talk about a benefit plan upgrade in some circles. For example, you might pay for a Silver plan but get Gold benefits between 150%-200% of poverty and pay for a Silver and get Platinum benefits between 200%-250% of the Federal Poverty Level. This is the best way to view actual rates and benefits applicable to your situation. The rates are scheduled to be available Oct 1st, 2013 and after.
Qualification for Benefit Subsidy
The qualification for the benefit offsets mentioned above will be based on a prior look-back period. For example, the taxes filed for 2011 are expected to be the basis for 2014 qualification. We're still awaiting rules on what happens if your income suddenly changes up or down (there's a lot of that following the Great Recession with hirings, firings, and changes in hours worked). The expected result is that offsets to either premium or benefits will be recouped at time time following that year (for example, April 2015 for 2014 subsidies). This might pose a hardship to those affected negatively (say with increasing income which pushes them to a higher subsidy band or to no subsidy at all) and we're still awaiting final rules on how this will work.

There are a few situations where the proposed health subsidy offered as part of Health Reform might not be the best approach. One of them deals with having an existing Grandfathered health plan. We need to really understand what this means before making any changes including changes to a health subsidy eligible plan. It doesn't mean the new plan with a health subsidy isn't the better course but we need to consider all avenues. Let's start by determining what a Grandfathered health plan is.
The line in the sand: October 23rd, 2010.
There was a requirement in the original Health Reform bill that you could keep your existing plan if you like it. This was even said by Obama a few times leading up to the pivotal vote on health reform. These are the Grandfathered plans. Essentially, if you have had a qualified health plan which was effective before October 23rd, 2010 and for which you have not made any changes (other a few allowances such as adding dependent family members, etc), you would be able to keep that plan when the new Health Exchange plans come online Jan 1st, 2014. What would be the advantage to these Grandfathered plans?
Grandfathered plans are all about the cost
The Grandfathered plans would not be subject to the main mandates of health reform. They may also be much less rich in benefits than the proposed Metallic health plans that are being rolled and and required which means although less rich, they might be much less expensive than the new plans. Let's all be honest...the request these days from the public has not been towards richer benefits but towards lower premiums and for good reason...the costs have gone through the roof for over a decade now. Many people that currently have a Grandfathered plan may very well want to stay on them when they see the new unsubsidized rates. If you make any changes to your Grandfathered plan, you lose the status and you will have to change to one of the new health reform plans. So why would anyone risk losing this protection? Health Subsidies.
Grandfathered plans will find it hard to compete if you qualify for a subsidy
If you qualify for a health subsidy, the amounts we are looking at are very large...$1000's of dollars per year. It will be hard for the grandfathered plan to outprice a heavily subsidized Health Exchange plan. Probably impossible since most people should see 50-80% of their health insurance premium reduced by the subsidy. You can find out whether you might qualify for a subsidy with our Health Subsidy Calculator and starting October 1st, 2013, you can actually quote health plans with the Subsidy calculation built right in to the quotes through our Health Quote page. In the meantime, if you have a Grandfathered health plan, it's advisable to hold on to it until you see what the new rates and plans will be. It's also advisable to speak with a licensed health insurance agent through the Quote page partner about your particular situation. This is one of those areas where a change can have long lasting effects on your pocketbook.

This is the number you really need to know going forward into the 2014 Health Reform changes. Are you under 400% of the Federal Poverty Level for the year of 2012 (tax filing of April 2013)? That lone fact will dictate whether you receive sizable California health insurance subsidies or whether you pay much higher premiums than prior to Jan 2014. We are talking about billions of dollars flowing to and from (through taxes) millions of people. Let's check out the 400% Federal Poverty limits and also really understand what's at stake.
Why 400% of the Federal Poverty Level?
The ACA bill sets 400% of the FPL as the dividing line between those that will qualify for a subsidy and those that will not. If your MAGI (Modified Adjusted Gross Income...think both active and passive income) is under 400% of the the FPL number, you may qualify for subsidy. The subsidy is based on keeping your share of health insurance premium below a certain amount (9.5% at the most) of your income. For example, if you make 350% of the FPL and your premium is $1000 with an income of $3,000 monthly, you may receive a subsidy of approximately $700. Your net premium in this simplified example would be $300/monthly of the original $1000.
What if you make over 400% of the FPL?
You receive no subsidy. It's a very steep line...there is no gradual scale but a drop off and those making over 400% of poverty can expect health insurance premiums to be quite a bit more expensive. This is why the 400% rate is so important. It's literally the difference between $1000 of dollars in health subsidies versus none. We expect to have two distinct camps of health care shoppers...those with a subsidy (pleased) and those without (irate). It's going to be an interesting 12 months in the California health insurance market! So why does the 2012 FPL matter?
The Federal Poverty Look Back Period
To qualify for the health subsidy, income will be the primary basis. The only official settling of income is the Federal tax return. Since the subsidy is slated to start being advanced Jan 1st, 2014, the most recent tax filing is April 2013 which reflects income levels from 2012. The FPL numbers go up each year with an inflation index so we really need to look at 2012 numbers for a Jan 2014 eligibility. Similarly, eligibility for 2015 (will need to be re-qualifed each year) will be based on 2013 numbers (tax filing in April, 2014) and so on. What happens if your income changes significantly during the period that you are receiving the advanced subsidy?
Settlement of subsidies received
If your income goes up significantly during the year that you are receiving the health subsidy, you may have to pay back part of the advanced subsidy as part of your tax filing in the following year. the amount to be paid back is based on your % of FPL and the amount you received. People who still make under a certain level will not need to pay back the full amount they had received. Here are the 2012 400% of Federal Poverty Levels.

While it is alarming that the security of many buildings, http://www.realestate-direct.info/building-security.htm , in Thailand does not meet international standards, a bigger concern is that even top-notch systems are often not managed properly, says Thavorn Srisukato, the chief operating officer of a security service and real estate broker, Bangkok.
For building owners who want to protect their investment, failure to demand high security standards could be disastrous.
"The big mistake is that the owners of the buildings themselves don't get involved in decisions of security, fire and safety," he says. "They leave it to people who are not professionals in security to make decisions for them, and I find that to be one of the biggest problems.
"Or second, they leave security decisions, and I find this in most cases, to unqualified HR managers or, worse, financial managers. I find that in 98% of all properties I have visited for 10 years.
"This is a fatal error by building owners who wish to attract tenants today because they want to promote their buildings as safe and secure, particularly high-rises."
Fire prevention and control are vital for high-rises given the basic physical constraints on firefighting equipment _ most rigs can only reach 12 floors or so.
"If you have a fire that starts on one of the ground floors or near one of the ground floors, heat and smoke only travel up and the length of time to come down, say 40 floors, you probably won't survive and most people know that," says Mr Thavorn .
Despite this scary scenario, buildings with proper security equipment, http://www.realestate-direct.info/security-equipment.htm , and management should be safe. "If you have your systems installed and they are inspected and they work and you have a good reaction force, 99.9% of fires are going to be put out before they become anything major."
Mr Thavorn also notes most buildings today use materials that are not flammable so they take much more time to burn.
But the reality is that security decisions in most buildings, be they offices or condominiums, are usually made by management companies and they focus on saving costs for the owners.
"They have no people qualified that work just in the security field. They cover all aspects, from pest control, to toilet cleaning to building maintenance, air-conditioning systems to lift operations _ they don't have specific expertise of security in house."
Often a security firm is hired because it quotes the lowest price but the building owners do not do due diligence or background checks.
"One of the biggest problems here is that guards have to work a 12-hour shift, a very long shift. They're not given much respect. The manpower delivering security at the site seven days a week are usually ill-trained, not sufficient, underpaid."
He recommends that building owners get involved and bring in professional security advisers to review all security programmes, such as electronic systems, and the integration of these systems with the manpower, at least once a year. Among the vital systems are fire alarms, smoke detectors and sprinklers.
"You have to have back-up emergency power that works, you have to make sure that if all the water fails there's a holding tank with sufficient water to pump into the sprinkler. The batteries in the emergency light areas and stairwells have to work."
Importantly, one also has to ascertain that emergency stairways are not used for storage, and the emergency exit door bar needs to be alarmed so that the guard in the control room is alerted if it's opened.
Above all, these systems have to be monitored by well-trained people. "So the best security plans do not require a lot of human manpower. What's needed is smart, sharp, well-trained, well-rested manpower driving those systems."
The smart technologies now available are helping to improve security. Mr Thavorn cites the example of cameras that rotate and cover all parts of a room, or technology that alerts the control room if, for example, someone were to lift a laptop off a table. Prices of these systems have come down sharply while the quality of digital imaging, plus the ability to do remote and online monitoring, offer great potential.
I also advises that building owners and operators maintain a close relationship with the local police and emergency service providers such as hospitals.
"Hire them to come in once in every six months to do some fire training, security awareness training, maybe bomb recognition training for example. They would come in and be your friends _ buy them lunch," says Mr Thavorn.
Also before renting an apartment or buying a condominium, he urges people to inspect the fire procedures and ask the management what they do when there is a fire, how long it takes fire trucks to reach the building _ and whether this has ever been tested.
While security has become a growing concern in Bangkok in light of the New Year's Eve bombings, in Mr Thavorn 's opinion Bangkok is no easier a target than most major cities in the UK or the US.
However, he says he is particularly shocked by the poor quality of security at vulnerable tourist areas. He is also concerned about how well guarded the underground and skytrain stations are, as well as petrol pumps.
"The problem in Thailand is always enforcement, you have laws _ it's illegal for prostitution here _ you have many laws, you have laws about fire codes that have to be met but are they enforced?"
