    While the Affordable Care Act (ACA) has garnered more than its share of controversy since it was signed into law, there can be very little disagreement about the fact that it has had a significant impact on the Commonwealth of Pennsylvania over the past three years. It starts with the health insurance marketplace established in every state. Of the approximately 1.2 million Pennsylvanians (12 percent of the non-elderly population) who are uninsured, about 1.1 million could qualify for either tax credits for health insurance or to purchase insurance directly through the exchange when it becomes reality in 2014.
     
    Refusal of Medicaid Expansion
     
    Pennsylvania's governor has refused to allow his state to participate in the ACA expansion of Medicaid, but a recent study conducted by the Pennsylvania legislature's Independent Fiscal Office estimated that the expansion could mean a boon of about $435 million for the state's treasury. Much of that money would be savings realized by the movement of residents from the state's medical assistance program onto the Medicaid rolls, which would be federally funded in full for the first three years, and 90 percent federally funded after that. Uncertainties about the continuance of federal funding have caused half of the nation's states to reject the Medicaid expansion plans, so Pennsylvania is not alone.
     
    Helping Young, Old and In Between
     
    One significant impact of the ACA has been felt by thousands of Pennsylvanians in their 20s, young adults who might find themselves without a job and health insurance after college. The ACA allowed people under 26 to stay on their parents' health insurance. As of the end of 2011, the most recent date data was available, approximately 91,000 young Pennsylvanians were taking advantage of this change in the law.
     
    And in 2012, thanks to a provision that closed the "donut hole" in Medicare Part D drug coverage, participating Pennsylvanians saved an average of $753 -- $167.6 million statewide. In addition, out-of-pocket costs for 1.034 million seniors in Pennsylvania benefited from the elimination of deductibles and co-payments for preventive and early-detection medical services.
     
    Price Controls and Rebates
     
    Nearly every Pennsylvania citizen, even those with their own health insurance through work or personal policies, is expected to benefit from the ACA cost control measures. The Act's 80/20 rule means that at least 80 percent of customers' premiums must be spent on health care costs and quality improvements instead of expenditures such as executive salaries. In 2012, that led directly to rebates for 575,551 Pennsylvanians at an average of $165 per family. In addition, Pennsylvania has received $5.3 million from the federal government through the ACA for programs that help guard against unreasonable premium increases.
     
    Other Benefits to Pennsylvanians
     
    Even without the expansion of Medicaid, Pennsylvanians stand to benefit extensively from the new health care law. Hundreds of millions of dollars have been funneled to the commonwealth in the form of research grants designed to help prevent illness and promote health. In addition, health centers in Pennsylvania received nearly $77 million in grants to support ongoing operations and to establish new sites, expand services and support major capital improvements.
     
    No matter where one stands politically on the Affordable Care Act, there can be no debate that the people of the Commonwealth of Pennsylvania have benefited already, and stand to benefit for decades in the future. The clincher: No longer will anyone be turned down because of a pre-existing condition. That alone has helped nearly 7,000 Pennsylvanians – the people who needed health insurance the most, but were ineligible under previous law – find a new lease on life in the Keystone State.

    For millions of people, modern technology seems too far away to garner. This is especially true for those that cannot afford to get a cell phone. For many, this seems like something that is rudimentary, getting a phone. Think about the cost of getting a cellular phone in these modern times, and you'll realize that it's not exactly an easy thing to procure. The cost of a phone can easily spike into the hundreds of dollars and that's not including the cost of service. The average phone bill today can be upwards of $100 for a single line, and that is usually compounded by a contract that lasts upwards of 2 years. Those that are less fortunate and dealing with financial hardships are not going to be able to get this option and therefore are left in the dark.
     
    The government has recently stepped into the picture and is now providing the poor with cell phone service and phones at no cost. Some people ask whether or not the government should be doing this, and the emphatic answer is a definite yes. Some pundits will be quick to say that this is a handout, but it truly isn't. The phone service is not mean to be abused and comes with a phone and 250 minutes of talk time. This is not nearly as much as many people assume that it is, and for those that are dealing with poverty, this is nothing more than a lifeline to a better future.
     
    Those that don't think about this often, take note of what can be done with a phone these days. When filling out a job application, one of the lines requires a person to list their phone number, but for the poor, with no landline and no phone, there is no way to contact them. This can be an issue, which is why the government is now subsidizing phones for the poor so that they can in fact get employed and moving forward. The subsidy is not without requirements, so it's not just anyone that can get this.
     
    In the available states, the cellular assistance program requires a person to be of lower income bracket and without recourse for purchasing a phone on their own. This means that they have to fill out proper forms and move forward with the process of procuring this option. Without proof of income and filling out the proper paperwork, this option is not available.
     
    For those that are asking whether or not the government offer free cell phone service to the poor, the answer is always going to be, yes. There are some things in life that are standard for communication and we have come to a modern place where giving those less fortunate 250 minutes of cell phone service and a free phone isn't going to break the bank and should be done to help them communicate with one another and get moving forward. There will be a time where a person that is given this device will be able to purchase a new one and get in line with many millions of other people that love technology, but until then, there are options that can help them.

    When it comes to Obama, a lot of people will either praise his work or will criticize what he is doing. It's hard to find people that can agree on everything in regards to politics, but there is one thing that he's doing that is helping millions, and no matter what political party you're loyal to, you'll find that there is greatness in regards to his dealing with communications.     For those that are skeptical, the facts are as follows:
     
    • Free Cell Phone – The phone is 100% free, for those that qualify. There are no strings attached here, 100% free, no hidden requirements, nothing at all.
     
    • 250 Free Minutes – 250 minutes of talk time per month with no additional costs at all. If you would like more, there are additional options that cost a low amount, but this is standard.
     
    • Income Requirements – Each state is different, but one thing is for sure, you have to have low income or be homeless to take advantage of this. You'll need to be able to prove certain things and fill out forms, but if you are within the required brackets, you will receive the aforementioned.
     
    The best thing about all of this is that everyone in the nation will be wired. No matter where you are and what you're doing, you can call someone and get help or wait for that job you've been hoping for to call and get moving into action. This is provided and subsidized by the United States Government and continues to help those that are in need. No matter what side of the political fence you're on, you can definitely agree that Obama is doing something great here with helping those less fortunate, communicate through cellular programs.

    One of the most under-appreciated benefits of maintaining a healthy weight and eating a balanced, nutritious diet is avoiding debilitating lower back pain. It's not just pain, either. The lower spine is the site where the largest and longest nerve in the body originates – the sciatic nerve. Compression of this nerve produces the set of symptoms commonly known as sciatica. These symptoms affect the lower back, the buttocks, the legs, the feet, and the toes. For example, a persistent sharp or shooting pain down the outside of one leg is often sciatica. So, too, is tingling or numbness in the toes of one foot, or a loss of weakness in one thigh muscle. All of this can shut down your ability to enjoy life, and all of it is far more likely to happen if you carry extra weight.
     
    Anatomy of the Spine
     
    Why does carrying extra weight – especially around the midsection – pose a significant threat to the stability of the lower back? Because the lumbar (lower back) region of the spine supports the bulk of the weight of the upper body, while also allowing for a wide range of motion such as bending and twisting. The spinal anatomy in that region is under particular stress, and is therefore more susceptible to the wear and tear associated with the aging process. Extra weight only serves to exacerbate the problem.
     
    But what does that mean, exactly? The stacked vertebrae within the lumbar region are home to the upper reaches of the sciatic nerve. As the body ages, the joints where the vertebrae articulate (facet joints) begin to lose their cartilaginous coating, threatening spinal stability. The body responds to the loss of stability in joints by producing osteophytes (bone spurs), which can threaten to impinge or irritate nearby nerves or the spinal cord itself. In addition, the intervertebral discs (sponge-like cushions between vertebrae) begin to lose elasticity, becoming brittle. Eventually, a disc or discs could begin to lose height or rupture.
     
    Why Obesity is a Problem
     
    These degenerative spine problems are generally worse in people who suffer from obesity. There are several reasons for this. To start with, the extra weight carried by people with obesity generates far more strain on the spine's bones, joints, cartilage, discs, and ligaments. Naturally, the more you ask your body's joints and other non-muscular structures to do, the faster they will wear down. And carrying extra weight only makes the job of the joints and other anatomical components that much more difficult.
     
    Also, people who are obese tend to have other physical trouble, such as poor circulation, respiratory issues, or a buildup of arterial plaque. These issues might be hereditary, but they also are often a result of poor dietary choices or a sedentary lifestyle.
     
    How to Combat the Problem
     
    Of course, as anyone who has battled weight problems for an extended period of time can tell you, setting the goal of maintaining a healthy weight and actually doing so are two different things entirely. That said, there are steps you can take to mitigate or avoid the potentially damaging effect of obesity on the anatomy of the spine.
     
    Start by having a conversation about the topic with your primary care physician. Explain that in addition to potential heart and respiratory problems, you would like to treat or avoid debilitating back pain associated with obesity. He or she will likely lay out a plan for exercising and eating a healthy diet in order to help you reach a manageable, sustainable body weight.

    Book your seats now if you're thinking about taking a one-day Grand Canyon rafting tour this Memorial Day weekend. The holiday is almost here, and these trips usually sell out fast.
     
    Anyone four years old and up can join in the fun – these aren't whitewater rafting trips and there aren't any rapids. Instead, they're gentle, smooth-water float trips.
     
    Grand Canyon rafting tours are all-day outdoor adventure experiences. Float tours are scheduled every day from March through November, including Memorial Day weekend. They begin at about 6 am, departing from Grand Canyon National Park Airport ("GCA") at Tusayan, Arizona (just outside the main gates at the South Rim) and Las Vegas.
     
    South Rim Tours
     
    At the South Rim, you'll have two tours from which to choose. The standard version takes you by bus from GCA to Page, Arizona. From there you'll go to the base of Glen Canyon Dam. That's where you'll board your pontoon boat.
     
    The deluxe version begins with a 45-minute plane flight from GCA to Page Municipal Airport. From there you'll hop into a Jeep for a terrific 4x4 tour of the "Slot Canyons" at Antelope Canyon. From there you'll head to Glen Canyon Dam and begin the river portion of your trip.
     
    Both versions treat you to a 15+-mile glide through the canyon's magnificence. You'll see some truly spectacular scenery, including Horseshoe Bend (picture red sandstone cliffs against beautiful turquoise water). The river portion ends when you arrive at historic Lee's Ferry.
     
    Tours from Vegas
     
    These float tours are favorites with travelers during Memorial Day weekend. They start with a 'copter ride from Vegas to Grand Canyon West. After the helicopter lands on the canyon bottom, the tour ends with a wonderful 11-mile float trip down the Colorado River. Total trip time is 10 hours.
     
    More details:
     
    About that helicopter ride – the West Rim is the only part of the National Park where choppers are allowed to fly below the level of the rim. After descending 3,500 feet to the bottom, you'll disembark next to the Colorado!
     
    The rafting portion of the tour begins at the base of mighty Hoover Dam, towering over 700 feet above you. You'll cruise through beautiful Black Canyon in a 19-passenger rubberized pontoon boat until you reach the end of the line at Willow Beach, Arizona.
     
    Come Prepared
     
    You'll be outdoors all day, so come prepared. The inner canyon can get hot during summer. I recommend bringing plenty of sunscreen and a waterproof bag with an extra set of clothes. Wear a long-sleeved shirt, sunglasses and a hat.
     
    All Grand Canyon rafting tours include lunch, drinks, the services of an expert river guide (who will share interesting info about native wildlife and plants), all necessary equipment, and all applicable taxes and fees. I'd recommend taking one to any traveler. Instead of worrying about planning what you'll do during your vacation, they let you focus on the experience.
     
    Summing Up
     
    Monday, May 27, is Memorial Day 2013. Most folks will treat it as a three-day weekend. The Grand Canyon is a popular holiday destination, and you'll have plenty of competition for seats on a rafting tour. I strongly recommend booking in advance.

    Henry Whipple (Dennis Quaid) is an aging man who is beginning to wonder what exactly his legacy will be before he retires. He grew up in a sleepy Iowa town as a third-generation farmer and salesman for Whipple Seeds, which sells genetically-modified seeds to area farmers. He wanted the same for his sons Grant (Patrick Stevens) and Dean (Zac Efron), but neither of the boys seems interested in following in their father's footsteps in "At Any Price."
     
    Grant went off to college and decided he wanted to see more of the world, so he dropped out to go climb mountains in Argentina. Dean stayed a little closer to home, competing in local stock car races in the hopes of one day getting into the NASCAR circuit. Henry doesn't understand his boys and has a rather contentious relationship with Dean, who doesn't seem to like his dad very much. Henry puts on a brave face and tries to act like a devoted and secure family man, even as he cheats on wife Irene (Kim Dickens) with the younger Meredith (Heather Graham). His fake smile is also hiding the fact that he is in danger of losing ground in the seed selling business to a rival, which makes him just desperate enough to make a few shady deals that could land him in hot water with the law.
     
    Meanwhile, Dean despises his dad for all the mistakes he has made over the years, but soon finds himself making some of the same mistakes. The one thing in life he never wanted to be was his father, but if he isn't careful, he may become just like him. Once this harsh realization hits, he has to find a way to change the trajectory of his life, just as Henry's life may also be changing. As Dean tries to make a better future for himself, Henry's future is very much in jeopardy due to his backroom dealings. Will the two reconcile and find a greater appreciation for each other, or will they both continue to squander once promising lives at any price?
     
    "At Any Price" is a well-written, well-acted film that is beautiful to watch. The beauty isn't just in the story or performances, but in the way that the Iowa background is used to perfection. Cinematographer Michael Simmonds captures the landscape in a way that is visually stunning to see, whether viewers have actually been to Iowa or not. The camera seems to constantly be floating over the scenic land, even as it is focused in on the characters as they have their moments of quiet desperation. It's beautiful work from Simmonds, who has worked with director Ramin Bahrani on all of his previous four films.
     
    Quaid turns in a very solid and affecting performance as the lead, who serves as both the protagonist and occasional antagonist. He is as much an earnest farmer trying to adjust to a rapidly-changing agricultural marketplace as he is a salesman who will occasionally engage in a few seedy (pun intended) deals in order to get a leg up on the hungry competition. Efron is equal to the task as his angst-ridden son who might have taken up almost any profession as long as he didn't have to work with his dad. The two play off each other really well, which could be because Efron is as handsome and charismatic as Quaid is. In a lot of ways, he is a younger Quaid with equally good acting skills and the same kind of devilish grin that melts women's hearts.
     
    Director Bahrani's films tend to focus on the American Experience as seen through the eyes of immigrants from various parts of the world. In "At Any Price," he takes a huge departure from his normal focus in order to show how the country's landscape has changed through the eyes of people who were born and raised here. There may be no better place to do this than the Midwest, where traditions and values are still very much the norm. As modern comforts like a forty-eight row seed planter clash with the old way of doing things in the film's small town setting, it becomes a metaphor for what is going on all over the country. Bahrani presents the characters and situations, but he is careful never to judge or make a pronouncement about which way is right, instead leaving that to the audience to decide. It's a nice, subtle touch that makes one wonder what Bahrani has up his sleeve for his next film now that he has started to break out of his comfort zone.

    As schools let out and temperatures rise, families and friends make plans to hit the cool, dark interiors of local movie theaters. Summer is the time when studios put it all on the line, casting nets of high-budget cinema in the hopes of drawing blockbuster crowds. Starting in early May, a number of anticipated films will make first-weekend runs, and there's a little something for every viewer's pleasure. The summer of 2013 is likely to be the summer of the superhero, the antihero, and the villain, with a few bits of extreme comedy, car racing, and apocalypse thrown in to keep things interesting.
     
    Three of the movies slated to appear between May and August feature favorite characters from comic books who are no strangers to the big screen. First up is "Iron Man 3," which hit theaters May 3. After three movies, the charmingly cocky Tony Stark is on top of the world. He's even added a bit of depth to his character and won his dream girl; when epic comic-book villain the Mandarin makes an appearance, Stark's world is torn apart in a few minutes of screen time. Viewers are likely to flock to theaters in the presummer season to find out if Iron Man can rise from the metallic shreds and put right all the wrong the Mandarin is doing in the world.
     
    How can writers make the gruff, well-explored character of Wolverine interesting to viewers for yet another X-Men inspired flick? The answer provided by "The Wolverine," which hits theaters on July 26, is to send him to Japan. As Hugh Jackman's portrayal of Wolverine has been a fan favorite since his first appearance, huge crowds are expected for this film. The Wolverine doesn't have a monopoly on fan love, though. Superman will make an appearance in "Man of Steel" on June 14. Although the superhero is always a crowd pleaser, this version of Superman is likely to be bolstered by Henry Cavill; women everywhere are already planning to see the movie based on the sexy Brit's role as Clark Kent/Superman.
     
    Heroes don't always come from comic books, however, and "Star Trek Into Darkness" will be bringing back favorites like Kirk and Spock on May 17. Much of the buzz around this film is related to Benedict Cumberbatch's role as the villain. Fans of BBC's "Sherlock" are keeping their fingers crossed, while Trekkies everywhere are ready to line up for tickets regardless of casting.
     
    The summer season is known for delivering family-friendly fare, and one heroic movie that will work for the whole family is "Percy Jackson: Sea of Monsters." Fans of the series of books by Rick Riordan will be delighted to see the young demigod back in action, and adults will be happy to take the family to a flick that doesn't involve constant booger jokes and high-pitched voices. The movie is scheduled for an early August release.
     
    Not all summer movies are family friendly, a fact that is amply illustrated by "The Hangover: Part III," which hits theaters May 24. Viewers can expect more raunchy fun and over-the-top humor as the infamous group of not-quite-adult males takes a road trip. Viewers who are looking for a different type of summer fun might be more interested in "Fast and Furious 6," which will be released on the same day. The cast includes numerous fan favorites, including Dwayne Johnson, Vin Diesel, Jason Statham, and Paul Walker; the all-star group is likely to drive fast and wreck cars for the viewing pleasure of numerous summer moviegoers.
     
    Couples looking for a date-night flick might consider "The Great Gatsby." The remake of this classic film had a May 10 release, and the excitement was bursting at the seams due to Leonardo DiCaprio's role as Jay Gatsby. If you like a bit more action in your date night, another option is "World War Z." The fast-paced zombie apocalypse flick stars Brad Pitt and tells the story of a plague of swift zombies that are unlike anything portrayed by popular zombie television.
     
    Another possible blockbuster is "The Purge," which is likely to be a hit with those who like their summer movies dark and disturbing. The movie, which hits cinemas on June 7, delves into what might happen if there were no consequences associated with crime. When all consequences are suspended for twelve hours and all regulatory and protective offices are closed, a family must fight to save their lives, their relationships, and their own beliefs.
     
    Dozens of other possible blockbusters are being released this summer. Every moviegoer will find something to spark the imagination in the dark cavern of the local movie theater. From heroes to villains, larger-than-life characters are coming to a theater near you this summer.

"The Reluctant Fundamentalist" also deals with the attacks, but does so from a relatively unique point of view. Instead of telling the story of the heroes of that day or the damage done to those who lost loved ones, this film shows the fallout of the attacks from the point of view of a Pakistani man who must watch the American dream crumble before his eyes.
     
    The film centers around Changez Khan (Riz Ahmed), a Princeton graduate who lands a job as a financial analyst on Wall Street soon after leaving school. By the time the film's opening ends he has left Wall Street behind him, having moved back to Pakistan and taken a teaching position at a university in Lahore. He agrees to an interview with an American journalist named Bobby Lincoln (played by Liev Schreiber) after an American professor is kidnapped in the city. It is during the interview we see what transformed Changez from a determined analyst in America into an almost anti-American professor in Pakistan who some suspect to have terrorist connections.
     
    Much of the film's story is told through flashbacks, starting with Changez' graduation from Princeton and recruitment by Jim Cross (Kiefer Sutherland) to a firm that specializes in corporate takeovers and downsizing. When the September 11th attacks occur and the initial shock has passed, Changez begins noticing that not only his life but the entire country seems to be changing. The attitudes of his coworkers and even people on the street change, he finds himself the subject of police and airport security harassment, and even his artist girlfriend Erica (played by Kate Hudson) betrays him by using his ethnicity to create a confessional, multimedia art installation.
     
    As if harassment and even being spit upon weren't enough, Changez' final disillusionment with the American dream comes when his company is hired to take over and shut down a well-known Turkish publishing house. The publisher had once published books of poetry by Changez' father, and seeing his company dismantle it and sell off the useful portions of the company makes Changez realize that his life is not what he had hoped it would be.
     
    The core story, based on the novel of the same name by Mohsin Hamid, presents a very unique viewpoint of post-9/11 America and how much of an effect it had on someone who had done nothing wrong but unfortunately shared a religion and ethnicity with the attackers. Unfortunately, the actual execution of the film took away from the depth and power of the story at times by overdoing the melodrama and flattening the characters into two-dimensional versions of themselves.
     
    Part of the problem is that there is a lot going on in the film, and too many plot points seem to compete with each other for attention. By the time Changez' company is taking over the Turkish publishing house it isn't clear whether that's simply the last straw after the horrors he had endured, or if he would have left that world behind at that point even if the September 11th attacks had never happened. The final act tries to wrap up the kidnapping plot that started the film but mostly just seems tacked on. Add to that a script that has as many ham-fisted moments as it does truly powerful scenes and some poorly-paced action sequences and you end up with a film that simply fails to live up to what it had the potential to be.
     
    The performances in the film are unfortunately as much of a mixed bag as the script. Ahmed delivers a solid performance despite some fairly dreary lines, but his work is somewhat overshadowed by the fairly lackluster performances of Schreiber and Hudson. During the interview sequences it's hard to picture Schreiber's character being any sort of journalist at all because he seems to lack a passion or even curiosity about the story at hand. Hudson's character just seems out of place in almost every scene she's in.
     
    "The Reluctant Fundamentalist" has a good story at its core, but too often that story gets lost in the film's failed attempts at building tension and showing the horrors of anti-Muslim hysteria. While it is not a bad film by any means, there are too many wasted opportunities and too much forced melodrama to put the film squarely into must see territory.

    After the international box-office success of "Star Trek" in 2009, Paramount and director JJ Abrams decided to make a sequel. It has taken four years to come to the screen, but "Star Trek Into Darkness" is finally set for a May 17, 2013 release. Shortly before filming commenced on the project, it was announced that British thespian Benedict Cumberbatch would play the primary villain, but no other information about the character was released until months later.
     
    Sometime after filming ended, rumors and speculation began to grow that Cumberbatch's character was Khan, the devious villain who was at the heart of "Star Trek II: The Wrath of Khan" in 1982. That film is widely regarded to be the best of the Star Trek films, with Khan serving as one of the most memorable villains from the series' television show and movies combined. It would be a big risk to have Khan come in as the villain, but Abrams has never shied away from taking risks.
     
    Abrams and screenwriter Damon Lindelof (who cowrote the script with Roberto Orci and Alex Kurtzman) were asked about the villain, but wouldn't reveal anything except to praise Cumberbatch in the role. Eight minutes of footage was finally shown to an enraptured audience at a Star Trek convention. Some of it featured the villain, who had up to that point only been known as John Harrison. Little else was known about the character except that he was a part of Starfleet and he had become disgruntled with the organization for unknown reasons.
     
    Star Trek fans the world over busied themselves looking up information in the canon about a character named John Harrison. They found that the name only appears as a very minor character in the original "Star Trek" television series. Fans tried to come up with a possible narrative that would explain why such an insignificant character would suddenly be the center of attention in a major Star Trek movie. Some of them thought that naming the character John Harrison was simply a ruse to try and trick them into thinking the villain was not Khan.
     
    It was announced that Paramount would reveal more footage at a convention in 2012, several months before the film was set to be released. The footage showed Harrison offering to heal the deathly ill child of a disheartened couple, which would seem to support the assertion that Harrison is highly intelligent. The couple had never laid eyes on Harrison before, so they asked him who he was; a question that everyone in the audience wanted answered as well.
     
    Neither Abrams, Cumberbatch, nor the writers revealed much about the character, but the trailers and snippets in the months leading up to the movie's release have begun to slowly reveal more about Harrison. One trailer showed that he knows how to pilot a plane, while another showed he is good at hand-to-hand combat. Yet another trailer shows two characters putting their hands together on opposite sides of a glass wall, which mirrors an iconic scene in "Star Trek 2: The Wrath of Khan." This tidbit and the fact that another major player from that film, Dr. Carol Marcus, is in "Star Trek Into Darkness" only serve to make fans believe that John Harrison is indeed a cover for Khan.
     
    Another trailer shows Harrison walking among the rubble of a fallen city that appeared to be a futuristic London. There is little doubt in viewers' minds that Harrison himself is responsible for the mass destruction, which is likely why Kirk and the crew of the Enterprise were called back to Earth. This sets the stage for a showdown between Kirk and Harrison, and Kirk clearly will stop at nothing in order to get his vengeance.
     
    Films are often shown to critics days or even weeks in advance of release, so they have plenty of time to write up their initial reviews. These can make or break a movie's opening-weekend prospects, but they can also provide minor or occasionally major spoilers for a film. These spoilers can also affect a movie's box-office performance, which is why most critics try not to include them in their film reviews. Unfortunately, in the case of "Star Trek Into Darkness," a few Internet critics have spilled the beans about Harrison and his ultimate goal in the film. This sort of takes the fun out of seeing it, so anyone who doesn't want to know more than the officially released info about Harrison should ignore spoilers and prepare to be surprised on May 17, 2013 when the movie is released.

    When the superheroes of "The Avengers" finally saved the world from Loki and his army, they all split up and went their separate ways. "Iron Man 3" (read more about the characters here) picks up after the battle in New York City, with Tony Stark (Robert Downy, Jr.) back in his hilltop mansion, tinkering with his Iron Man suit. On the surface, things might seem fine to some, but Stark is definitely not fine. He has long, frequent lapses in concentration that are driving his girlfriend and business partner Pepper Potts (Gwyneth Paltrow) crazy. He is clearly traumatized by what happened, but he has no real outlet to try and get it all out.
     
    Unfortunately, he won't have time to really recover because forces are already in place to make his life miserable. The Mandarin (Ben Kingsley) has plotted to kidnap and murder the President (William Sadler) among other nefarious plans. Meanwhile, Aldrich Killian (Guy Pearce), who works for offshoot AIM, plans to use a new program he helped develop called Extremis to heal amputees. The only problem is that if the patient's body rejects the Extremis treatments, they end up exploding as a human firebomb. Killian doesn't seem to care about the deaths, instead forging ahead with plans to use Extremis in a way that could threaten a lot more than just a few unwitting test subjects.
     
    When Tony publically takes a stand against the Mandarin and calls him out, his mansion gets destroyed and he nearly drowns in the ocean. Taking advantage of his presumed-dead status, he lays low in a rural Tennessee town to get his bearings. What follows is a series of rescue missions that are a complete thrill to watch.
     
    "Iron Man 3" is the first film in what Marvel Studios calls Phase Two of its movie plan. It sets up this phase quite nicely by giving an honest look at how being a superhero charged with saving the world can really take a mental and emotional toll on someone. It is an unflinching look at the humanity of Tony Stark when he is outside of his suit, and it is a great way to reintroduce him as a solo character. Though Stark was given plenty of screen time in "The Avengers," it is nice to see him as the center of attention again, especially when he begins to get his mojo back and changes from Tony Stark to Tony Snark. The jokes and quips that fans have come to know are back, and so is Iron Man in all his glory.
     
    The script, cowritten by director Shane Black and Drew Pearce really breaks Stark down to his lowest point in the series, only to build him back up, better and wiser than before. It is not unlike what "Skyfall" did with the James Bond character, similarly sinking Bond to his lowest level so that he could rise to the occasion, better than ever. Stark will need the extra oomph, because he has to pull off a series of amazing rescues, each one more daring and thrilling than the last. The fact that a pre-Avengers version of Iron Man might not have been able to pull off these feats shows just how much attention has been paid to character development in the series. Since the original "Iron Man," Stark has gone from an overgrown child with too much power and money on his hands to become a real hero who respects those around him, even if he does crack one-liners at their expense.
     
    When Downey, Jr. signed on to do the first "Iron Man," he also agreed to appear in several more films, including sequels and the eventual Avengers project. With "Iron Man 3," he has officially completed his contractual obligation, meaning that there is a possibility that this may be the last time audiences see him suit up. Marvel Studios head Kevin Feige has said that he is confident the studio can sign him to another contract to continue on as Iron Man into the future. "The Avengers" scribe Joss Whedon, who is currently penning the sequel, has said that he is writing the script with Tony Stark in it. With the actor keeping mum about his future plans regarding the franchise, if "Iron Man 3" is the last time Stark and Iron Man are on the big screen, then it is a great way to go out.

    The inspiration for this article came from a discussion I had with a close relative of mine. To place the conversation into context I need to give you some back ground while keeping names and actual relationships out of the text. So this relative of mine, we will call her Jane, has two grown children we will call Sally and Sam, both over 30 years in age. Both Sally and Sam have children of their, Sally is in college and works full time to support herself and her children. Sam on the other hand never really did much in the way of continuing education after high school and never really held a full time job. He and his 3 kids live in his mother's house and it is his mother and father who bare most of the cost for most of the living expenses for all of them. Does this scenario sound familiar to anyone?
     
    During our discussion I learned that Sam's ex-wife had told Sam that it was his mother's fault that Sam never attempted to further his education nor ever really put any serious effort in finding a real job. She told Sam that the way his mother raised him and the way she makes his life too comfortable for Sam is the reason he never really tried to change his life. To say the least, Jane took just a little bit more than a small exception to the statement, but is there any truth to what Sam's ex-wife is saying?
     
    This brings up three old arguments, old at least from my point of view because I have heard them in my home all my life. The first is Nature vs. Nurture, next would be Help vs. Enabling, and last being where to draw the "Line of Responsibility". Unfortunately to cover all three adequately will need to break this into three parts. I also encourage you to visit us at DragonTamers.com for further details and discussions.
     
    As much as I care and respect Jane, I have to say that there IS some truth to Sam's ex-wife's claim, but only some.
     
    In part one I am going to focus on Nature vs. Nurture. Basically the debate discusses why a person grows into the adult they become, is it enviable because we are a result of our genetics (Nature), or is it a result of the environment we grew up in (Nurture)? The basis of those favoring the side of environmental was a concept called the Blank Slate Theory. In this theory every child born is a "Blank Slate" with no predispositions toward any type of behavior. This theory subscribes to the concept that if you control a child's environment, what they are taught, what they are exposed to from the outside world, and even what behavior from their parents they are allowed to see, then one can shape the person that child will grow up to be. The hard liners of the Nurture argument indicate that if a child grows up to be a noble prize winner or serial killer is direct result of how they were raised.
     
    On the Nature side of this argument, it is believed that all folks are born with a genetic predisposition toward a certain personality, usually a blending of elements from the personalities of the members of the genetic line that the individual inherited. That would be parents, grand-parents, etc... The hard liners of the Nature argument basically indicate that who a child becomes when they reach adulthood is already hard coded into their genetics and would be evitable despite the factors in a person childhood.
     
    At DragonTamers, we believe that both arguments are valid to a certain extent, with the strongest influences being those of nature. However I must point out that one factor not accounted for in this argument is Choice or the free will of every individual.
     
    Science has long ago proven that the "Blank Slate" does not exist. There are many studies that prove a strong genetic influence in who a person becomes. Some examples are studies showing how identical twins separated at birth tend two grow up having very similar lives. This occurs even when the twins are raised without contact with each other and in families with very different family values. These identical twins more often than not make the same or very similar career choices, like the same foods, even end up marrying (or not) someone very similar in both personality and appearance to the other. Other studies have shown how sons who grew up never meeting their natural father will often display personality traits and world views much more in common with their biological fathers, then the men who raised them.
     
    At this point science is seemingly on the Nature Side of the argument, but as they say, "that ain't all she wrote". Science has also shown clear evidence for the influence of Nurturing. The FBI for example has shown that they can prove that most serial killers do have a history in common of being sexual abused (a broad definition that includes age inappropriate sexual contact with an adult), even if in the minds of the killers they were not being abused at the time.
     
    Certainly the FBI studies are the extreme end of spectrum, but much closer to home I used to work with a fellow who belonged to a particular religious sect. This group leveraged the state laws on home schooling to pull of all the kids of the sect member families from the school system. These kids would then under the guise home schooling attend a NON-Sanctioned religious based school. The information and concepts that children were allowed to be exposed to was very limited to just what they had in the school. They didn't have TV, Internet, or radio at the homes. These kids were only allowed to associate with the kids from the families of the sect. While I was shocked to hear his description, I had the presence of mind to ask him a few questions on how he handled this. The first was that didn't home schooled kids still need to pass the standard states tests. He then told me that they wait until about a year out from the tests to "prep" the kids with the questions. By then the kids have been comfortable with the "truth" long enough not be confused by the heresy the test prep exposed them to.
     
    What my friend and his school were doing was taking advantage of a well-documented phenomenon that we at Dragon Tamers call the "Just Is"; in a nutshell this is where young minds accept what they are exposed to as truth without need of evidence or convincing. Studies show that concepts and beliefs formed during this time of a person's life tend to be retained as truth, even after the individual is exposed to overwhelming evidence contradicting these "truths". So it would seem science also supports the nurture side of the argument.

    In part two of my three part article I would like to focus on Helping vs. Enabling. It seems almost like a sign of the times that more and more families find themselves with one or more adult children who are having trouble getting started in life. Often these adult kids end up living with their parents, sometimes beyond their 30th birthday.
     
    Talking with parents in this position if have found that the situation they are dealing with have a great number of similarities with each other. In most cases the parents have raised multiple kids, most of whom have gone on to have relatively successful early lives. However they find themselves dealing with one child that just can't seem to get their act together. The details of the reasons are as varied as the leaves you have in the yard during the fall, but the actual reasons are remarkably similar across the board.
     
    The common thread seems to be that as some point in the child's growth, their mind (maturity) stopped growing but their body continued. Now you have someone with the heart & expectations of an unruly teenager with a body of an adult and all the legal rights that brings them. Some cases these Adult Children even start having children of their own, which really makes things complicated for the parents who are still basically caring the adult child. The wellbeing of the grandchildren usually become great leverage for the adult children to use to continue in the life style they have become accustom to…
     
    First, before we start talking about what to do about the situation, I first need to address a large concern of most parents in this situation. Almost all what to know the answer to the question – this is our fault?
     
    The truth is, mostly no... I say mostly because the cause of situation was only somewhat within the parents control, the situation was a result of the child's perception and instinctive reaction. The continuation of the problem is because the parents allow it to continue. I invite you to visit DragonTamers and review the posting on the PIFTB Cycle. As you will see, the B – Standing for Behavior comes about as a result of the combination of Perception AND the Instinct. Remember Perception is shaped by experience and education, perception changes over time, and perception can be influenced by parents. What parents cannot change is the Instinct which is a result of the genetic predisposition the child has from the moment of conception. Bottom line, as a parent you can do everything right by the best information available today, but still you end up with a 30 year old, unemployed child with kids living in your home, usually rent free.
     
    So if you are dealing with this the question you most likely have is what do you do to fix it?
     
    That is a complex question, and the first step to resolve this issue is to understand if you have a Helping or Enabling issue. Sometimes folks really NEED help, but more often the help you give just allows the individual from needing to make changes in their life. So how do you determine if you are helping or enabling? Well the rule of thumb is pretty simple. Ask yourself this, has the help you given to date resulted in steady and progressive positive changes in the individual's life? Or have you do you find that your relationship is back to the same place, or even worst over and over again?
     
    If you believe that all your efforts seem to have little or no influence to change the behavior of the adult child, it is more than just a little bit likely that you are Enabling this individual. If that is the case, what do you do? First remember that the moment to moment environment is a result of our choices. Free will exists for both the parent and the adult child. Unfortunately the child has place their own free will in a box and have embraced a life of "what they feel like" where what they want is more important than anything else. In these situation parents have the right and responsibility to exercise their free will and just say no. That is a great place to start. What we find in Enabling situations is that the adult child will not be denied the things they want and believe they need. If the parent does not provide it, the adult child will find it some other way to obtain it. Humans have fantastic instincts which have not only allowed us to survive but to exceed. So rest assured, if you cannot reach them through reasoning, they have all the necessary instincts inside to survive.
     
    In part three of this series I would like to discuss at what point the responsibility of the parent or another individual providing the help ends and what point it begins with the adult child.
     
    Coming back to my relative Jane and her son, Sam we talked about in part 1.     In Part 3 I would like to discuss what we call at Dragon Tamers as the "Line of Responsibility". The line is the point where one can see that responsibility of one person ends and the other begins. If you have not already done so, please do a search for the first two parts titled "She Blamed What on his Mother? Part 1 of 3" and "She Blamed What on his Mother? Part 2 of 3" and give them a read. This will assist in understanding the context of part 3.
     
    One of the topics that cause great amounts of misery for people is that they do not know when they should be dropping responsibility and also when they should be picking it up. As we discussed in parts one and two, Jane's was distressed over the accusation that is was her fault that her adult son of 30 years with 3 children never held a real job, did not continue his education after high school in any format, and still live at home with her and her husband. In part 2 I asked readers to post comments on their opinion as what Jane should do. As those come in, I would like continue and discuss at what point is Jane and her husband no longer responsible.
     
    The Law states that a child reaches the age of consent at 18. They are granted or gifted the privilege (called a right) to vote, legal status of choice in their life, but with the exception of the requirement to register for selective service, the law does not really require much in the way of responsibility. That is the funny thing about that age, all legal responsibility is completely conditional, and in some cases optional for 18 year old adults. Do not down play the importance of these as environmental factors for the adult children. As many parents can testify, some adult children focus on their "rights" and have been for years. More than one parent in this situation had to deal with their 16 year old dropping out of school because the law allows it, only to spend the next two years parked in front of the TV because the law also placed the responsibility for the child's wellbeing and actions on the shoulders of the parent.
     
    In the real world a Parent feels responsible to "raise and guide" our children all of their lives. However parents for both their sake and happiness and that of their children need to learn to let go. Children as they grow in most cases will naturally take on responsibility for themselves, which of course makes it easier and natural for parents to find the Line of Responsibility. However in this modern world, people, including children receive a large amount of data. Much of this is marketing propaganda about being in touch with one's feelings (people who embrace their feelings are much more likely to impulse buy) and political propaganda that claims that one is not responsible for their own lives. These are in fact powerful factors of the environment that children perceive.
     
    To really understand the Line of Responsibility one must understand Freewill. We all have heard of it, yet most responsible people fail to acknowledge it. Every person has freewill, and at the age of 16 we start to legally grant children the privilege to exercise free will. The Line of Responsibility goes hand in hand with Freewill. See the Line of Responsibility is drawn at the same place in a person's life that they obtain the right to exercise Freewill. After all is not an exercise of Freewill when an Adult Child makes the choice not move forward in life?

    I want to ask you two important questions. Do you own a home safe filled with cash, silver & gold? If not, what are you waiting for? As an American, most of us cannot really imagine making the same statement as the title, however if the warnings of many experts comes to pass, this statement may become an everyday reality. Every day we see more and more examples of economic distress both on a national and global scale. While to most of us, these events seem to be very distant and have nothing to do with us personally; unfortunately this cannot be further from the truth.
     
    We live in a global economy, and what happens to one country affect us all. All you need to do is watch the news and you can see for yourself that both the United States and World Governments are in serious financial trouble. The latest the news is reporting that the European Union is looking to bail out the small country of Cyprus, using funding obtained by "Taxing" the savings accounts of individuals. Another story as of 31 March 2013, any account in an Australian bank that has not had owner activity in three years can now be legally confiscated by the Australian government.
     
    Closer to home, the US government has been "Examining" the way Americans are using the $19.4 Trillion they hold in retirement savings. Under the guise of protecting the elderly from scams, the US government is considering ways they can "manage" these assets. Note: The national Debt is over 14 Trillion. Imagine if you would how neatly it would work out for the United States Government if they could "Borrow" that 14 Trillion from a US Government Managed fund, and then be in debt to a US Agency instead of foreign powers. Since this US Agency would be funded by managing retirement accounts, funds can be paid out over time at a controlled rate, and controlled interest rate. Let us not even consider what would happen to funds of folks who passed away before all of their "entitlements" are paid out. Just food for thought.
     
    As scary as this sounds, this actually NOT the worst thing that can happen.
     
    Since 1944 the United States has held the most favored world economic position of having the United States dollar being the World Reserve Currency. This placed the United States as a nation and American Business at a strategic economic advantage. An advantage that our government has failed to leverage in an advantageous manner for the United States and in fact has placed the economy of the entire globe in jeopardy. History has shown that in time of crisis, everyone starts to look out for themselves, this certainly includes foreign governments. Indications make it clear that the world governments consider one of the most advantageous actions they can do to protect themselves economically is to drop the US Dollar as the World Reserve Currency. The process has already started and this event seems enviable. But what does that mean should it come to pass for American Citizens?

    When I started my research on this article I had some solid preconceived ideas of the information I would find when it comes to gun violence. What I had set out to do was to dig up data and present the argument that gun owners need to do a better job of securing their weapons. Based upon a number of personal experiences I had with certain gun owners, I felt that I may have had a first-hand understanding of how some many illegal guns make it to the street, and why gun control advocates felt they had legitimate issues with gun owners. When two of my associates ended up having their guns stolen from their home during a day time robbery that occurred while they were at work, I figured this was a major source of illegal guns. What I found instead was a world of political posturing, half-truths, and clear avoidance of the real issues.
     
    During my research I came across several articles discussing six points of concern about guns. I have them listed below in the order I address each one.
     
    1) Guns are used in more than 70% of homicides (actual number is between 70% and 71% to be clear).
     
    2) Guns are used in most suicides, about 52%.
     
    3) Guns are typically used in mass killings.
     
    4) Guns take only 5 days to buy (in some states, more in most)
     
    5) Guns purchased at gun shows can be taken home immediately (In some states, but not for residence of states having waiting periods)
     
    6) Guns take innocent lives.
     
    I would like to start off with a bit of information from the horse's mouth shall we say. Gun control advocates would have common folks believing that because we have legal gun ownership, hand guns are used to commit crimes. However, according to the ATF (Alcohol, Tobacco, and Fire Arms), only 10% to 15% of legal firearms are used in criminal activity. Let me say that again… 10% to 15% of LEGAL firearms are ever used in criminal activity. Another way of stating this is that 85% to 90% of firearms used in criminal activity are obtained ILLEGALLY. Wow, that number surprised me. See while I never really bought into the gun control advocates position that removing the guns from the hands of legal owners would stop gun violence and crime, I thought that the statistics would at least somewhat justify that claim. According to the ATF, lost or stolen legal weapons are at the bottom of the list of sources of fire arms used in criminal activity. Please note that the 85% to 90% of weapons obtained through illegal sources does not include the theft of legal weapons (an illegal or criminal act). It would seem that these statistics do justify a call for strong action, that action being to stop illegal gun trade, not private citizen gun ownership.
     
    Bring this back to point number one of the six concerns of guns, 59.5% to 63% of homicides are committed by guns obtained through illegal trafficking. 30% of homicides are committed NOT using a gun, and the remaining 7% to 10.5% are committed using guns obtained legally.
     
    In my second example my research turned up that most suicides (52%) in the US are carried out by guns. That is something to think about. However Gun Control proponents present the case that this data provides a valid reason to overturn the right of Americans to own guns. The logic being is if private citizens were not permitted to own guns, then the statistics would be different. The truth is, yes if we did remove the right of private citizens to own gun, then the number of suicides committed using guns would drop. Drop but not disappear since it stands to reason that some of that 52% is committed with illegal guns; guns that would still be present after removing the right of private ownership. So we while it is clear that the number of suicides committed by guns would be reduced, the really important question would be whether the NUMBER of suicides would be reduced.
     
    Examining this question I did a search on suicides rates worldwide and found some interesting results. The Untied States rate of suicide is number 33 on the list, with Greenland as number 1 and with a rate per person nearly 10 times that of the United States. In Greenland the method of choice for suicide is hanging at a rate of 46%. The wise guy in me would like to recommend that in the US we ban guns and in Greenland they ban rope, that should save lives right? All sarcasm aside, looking at the 32 nations who demonstrate higher suicide rates than the US, many like Greenland use means other than guns to take their own lives. Even close to home, 48% of suicides are NOT committed by guns. So it is clear that the presences of guns MAY have an influence on the method a victims selects, but it appears not to have an influence on the actual number of victims. If one is REALLY concerned about saving lives, they should be focusing on ways to turn people away from suicide, not debating gun control.
     
    In point number two we state that guns are used in 52% of suicides in the US, but I can find no evidence to support the idea that removal of legal gun ownership would reduce the loss of life due to suicides.
     
    So it has been stated that guns are typically used in mass killings. Examination of this claim has found some truth to it. If we limit our data set to just the last century, AND we remove all mass killings performed by governments AND terrorist, we find that guns are involved in 52% of the cases. Studies show that the number of victims on average when a gun is used is 4.92, just a little better then knives, blunt objects and bare hands with an average of 4.52 victims per incident. Guns effectiveness pales in comparison to fire with average of 6.82 victims per incident, but nothing beats explosives that deliver an average of 20.82 victims per incident. Examination of mass killings in the US during the last century shows no relationship between availability of guns of any type with the frequency and devastation of mass killings. Social scientist believe that the rise of spectacular mass killings and terrorist activity both in America and worldwide is linked to availability to mass media. In both cases the motivation for the murder is not to kill the actual victims but to reach the larger audience available only via the media.
     
    One of the most interesting observations I came across while researching this article was the way the same data was used by both sides on the issue to support their position. Truth is only true when all the data is presented, otherwise it is just deception.

    Emergency room physicians are accustomed to treating many types of illnesses, accidents, and other ailments for patients of all ages. Pediatric patients can be some of the most challenging in the emergency department, partly due to the types of mischief they may get themselves into, and partly due to their decreased ability to communicate clearly about what is ailing them. Children are also prone to curiosity, which often leads to accidental poising, choking, and similar events. Sometimes, accidental ingestion of objects like marbles, keys, or coins can be handled quickly if the parent sees the object and removes it quickly from the child's mouth. However, in cases where the object is swallowed completely, life-threatening complications can result.
     
    A group of emergency room physicians at a hospital in California saw so many cases of accidental coin ingestion that they developed a special team of ER physicians to handle the events. While most parents know that children enjoy chewing on or attempting to eat small objects, they may not realize that coin ingestion in particular is extremely common. If a coin is swallowed, it most often passes through the entire digestive tract and passes out of the body harmlessly. However, at other times, the coin can become trapped in the esophagus, leading to throat irritation, gagging, or vomiting. In the most severe cases, a coin can cause esophageal perforation, migration (ie movement into other areas), or a tracheoesophageal fistula (an abnormal connection between the esophagus and the trachea). Some cases are fatal to their victims.
     
    At the hospital discussed here, emergency room physicians had typically passed cases of coin swallowing to pediatric surgeons, pediatric gastroenterologists, or otolaryngologists at their institution, rather than handling the surgical removal themselves. However, in 2004, the hospital decided to train its emergency room doctors to perform the procedure(s) themselves in order to make care faster and more efficient. The physicians and hospital then documented the success of this strategy over the following years. The patients included in the study were those younger than 18 years who had ingested a coin within the preceding 72 hours; in most cases, the coins were located in the upper third of the esophagus. The emergency room physicians who performed removal on these patients followed established guidelines, such as using rapid sequence intubation (RSI) followed by direct visualization of the esophagus with a laryngoscope. In cases where the coin was visible, the physician would then remove it using Magill forceps (a type which are curved to allow better access to the esophagus). In cases where the coin was not visible, a Foley catheter was passed beyond the coin, its balloon inflated, and then the catheter was withdrawn.
     
    In addition to the data about the outcomes of the procedures they performed, the physicians also noted statistics on which types of coins were most likely to be swallowed, and general descriptive statistics about patients. For instance, most of the patients were male, and their average age was 19 months (with an age range of 4 months to 12.8 years!) The most commonly swallowed coins were pennies, followed by quarters, and then nickels. On average, it took about 8 minutes to remove the coin from each patient; in cases where the physicians could not locate the coin visually beforehand, it took about 12 minutes to remove. Most patients had only very mild complaints following their treatment, such as sore throat, dental pain, or a mild cough.
     
    Overall, the decision to train emergency physicians to perform esophageal removal of coins was viewed as a success. In the 101 cases that were included in the study, the procedure was successfully carried out 95% of the time. The authors note that while they made sincere efforts to follow up with the families of children who had undergone the procedure, not all families participated in the follow up questionnaire or interview portion of the study, meaning any complications they may have suffered were not recorded. Furthermore, any children with resulting complications from the procedure that were treated elsewhere were not reported, again potentially limiting the data for this part of the study. Despite these minor facts, the study was successful and the hospital where it was carried out plans to continue using emergency room physicians for pediatric coin removal cases. This particular study along with the training it described are good examples of how emergency medicine jobs may involve more than just routine care for heart attack victims or severely injured patients. Often, these doctors may encounter extraordinary and unusual situations, and must respond quickly and effectively.

    Sciatica symptoms refer to the problems that can develop as a direct result of the compression of the sciatic nerve. Not a condition in and of itself, sciatica simply refers to the pain or discomfort that can occur because of a degenerative spine condition or other issue in the body. As a result, effectively treating sciatica is contingent on identifying what is to blame for the sciatica in the first place. This can be somewhat troublesome because all too often, individuals waste valuable time by attempting to treat the symptom – sciatica – while neglecting what is actually causing the persistent discomfort.
     
    The sciatic nerve is not only the longest nerve in the body, but at an inch thick, it is also the widest. Stemming from the base of the spinal cord in the lumbar spine (lower back), this nerve traverses down through the buttocks, hips, and legs, before culminating in the feet near the toes. In short, the sciatic nerve is exceptionally important and absolutely integral in our ability to walk upright. However, the location of the sciatic nerve in the lumbar spine can also be somewhat problematic.
     
    As we grow older, the wear and tear of years of regular movement takes a toll on the spinal column. This happens at every level of the spine but is particularly common in the lumbar spine, where the burden of supporting most of the body's weight and the extensive flexibility of the lower back can accelerate deterioration. This spinal degeneration is an entirely normal side effect of the aging process and explains the loss of flexibility and minor aches and pains that most people accept as being a standard part of getting older. However, when degeneration advances to the point that the sciatic nerve is affected, the symptoms of sciatica can arise. This can be caused by everything from herniated intervertebral discs, the arthritic deterioration of vertebral facet joints, the presence of anatomical material in the spinal canal, vertebral misalignment, and any number of other conditions. Furthermore, injury can also lead to sciatica both in the immediate aftermath of the injury, as well as in the years to come as spinal deterioration advances.
     
    The specific sciatica symptoms that can develop vary from patient to patient depending on the cause of the problem, the location of the compression, the patient's overall health, and other mitigating factors. Localized back pain, pain that seems to radiate along the sciatic nerve, numbness or tingling in the feet or toes, fatigue or weakness in the lower body, and sharp leg pain are all common examples of symptoms that many people deal with when stricken with sciatica.
     
    Frustratingly, the compression of the sciatic nerve can be difficult to diagnose because of all of the potential causes and because it can lead to a number of different sciatica symptoms – many of which may seem unrelated to a problem in the spinal canal. For instance, when the regular function of the sciatic nerve is interrupted, the body might respond by sending pain traveling along the length of the nerve or even cause numbness and tingling in the extremities.
     
    The good news about sciatica symptoms, however, is that once the cause of the problem is uncovered, there are a variety of different treatment options that might be considered, depending on the patient and their overall health. While treatments vary from patient to patient, most benefit from a regimen of conservative, nonsurgical techniques over the course of several weeks or months. In other cases, surgery is occasionally recommended to treat the source of the patient's sciatica.

    One of the fast-growing healthcare professions in the United States is nurse practitioner. A proliferation of urgent care clinics and wellness clinics located in some drug store chains has created a demand. More than 600 million visits to nurse practitioners are made by Americans every year, according to the American Association of Nurse Practitioners™. While a nurse practitioner is not a substitute for a physician, per se, a nurse practitioner can and does serve many of the same functions as a family doctor or specialist.
     
    What is a Nurse Practitioner?
     
    A nurse practitioner (NP) blends an earned expertise in diagnosis, treatment, and ongoing wellness to help his or her patients. An NP often works in rural, urban, and suburban communities, at hospitals, clinics, emergency rooms, primary care practices, and public health departments. Additionally, an NP typically works autonomously and in conjunction with other healthcare professionals, such as physicians, surgeons, nurses, physician assistants, radiologists, and more. They may prescribe medication, order diagnostic tests, and perform most of the same functions as a physician.
     
    'Whole Person' Health Care
     
    One thing that sets nurse practitioners apart from other types of healthcare professionals is an emphasis on the health and well-being of the whole person. In addition to diagnosing existing health issues and administering treatment, a nurse practitioner will help a patient by providing advice and counsel on how to remain healthy moving forward. A nurse practitioner will also provide guidance about lifestyle choices (diet, smoking, exercise, etc.).
     
    What Type of Medicine do they Practice?
     
    They are active across a broad range of patient populations, including acute care, adult care, family medicine, gerontological, neonatal, oncology, pediatric, psychological/mental health, women's health, and more. The largest percentage of nurse practitioners work in family medicine with a primary care focus, while the most experienced nurse practitioners tend to be those who focus on women's health in a primary care setting. Nurse practitioners also are active in a large number of subspecialties, including allergy and immunology, cardiovascular health, dermatology, emergency medicine, endocrinology, neurology, sports medicine, urology and many others.
     
    Education Requirements
     
    After completing a professional registered nursing program, in order to advance to nurse practitioner status, a candidate must compete a master's or doctoral degree program. Clinical courses help prepare nurses who already have attained a certain level of experience with particular specialties (nutrition, for example, or orthopedics) to provide primary care. There might also be courses and training designed to prepare nurses to provide acute care and long-term care.
     
    In addition to formal education, a nurse practitioner candidate must undergo a rigorous program of national certification, which typically includes periodic peer review, evaluations of clinical outcomes in cases supervised by the candidate, and a demonstrated adherence to a code of ethical practice. There also are a number of continuing education and professional development programs in place to further prepare a candidate for a nurse practitioner position.
     
    A Promising Future
     
    An aging population in the US and dramatic changes related to the Affordable Care Act will help increase demand for nurse practitioners. Even though it still requires a rather significant commitment of time and money to secure credentials of a nurse practitioner, that credentialing process pales in comparison to the eight or more years typically required to become a specialized physician or surgeon. That's only one of many reasons why the career of nurse practitioner is projected to grow in the double figures, at least, over the next decade. While salaries are not projected to surpass those of practicing primary care physicians any time soon, the sheer demand for nurse practitioners is almost certainly going to continue to drive pay in a positive direction over the long term.

    I've always had a soft spot for injectable emulsions, probably because, I spent most of the first 8 years of my career formulating insoluble drugs into emulsions for IV administration. But 50 years after the European approval of Intralipid, propofol and clevidipine remain the only two non-nutrient drugs I know of that are formulated as injectable emulsions. In the last few years we have seen a significant uptick in approved cyclodextrin-solubilized products, and the use of anaphylaxis-inducing cremophors and polysorbates persists, but emulsions remain swept in the corner and all but forgotten.
     
    The biggest appeal of injectable emulsions as drug delivery vehicles is that they are composed entirely of stuff that is floating around the bloodstream anyway: triglycerides, phospholipids, water, and a bit of glycerin for tonicity. To the body, the oil droplets look pretty much like chylomicrons, and they are metabolized and cleared as such. Other solubilizers, be they cyclodextrins or PEGylated surfactants, are not things nature intended us to have in our blood, and there are consequences to this, be it the potential for nephrotoxicity with cyclodextrins or the risk of anaphylaxis with the surfactants.
     
    That is not to say that emulsions are squeaky clean. If you give too much too fast you will quite obviously see spikes in serum triglycerides, but, more ominously, increases in liver enzymes can also occur. In practical terms this means that an emulsion formulation might be a good option if it will be administered as a relatively small volume, but if hundreds of milliliters per day are needed, then lipid intake could be a problem. Of course if you need to give that much of another solubilizer, you might avoid the liver toxicity but get hit with something else; it's tough out there in the world of infusing insoluble molecules.
     
    So what is so unsexy about emulsions? They are hard to make, but so are liposomes, and liposomes are still pretty hot stuff after all these years. But, of course, liposomes can do so much more. They can be used to encapsulate water soluble drugs, can be pegylated to hide them from the big, bad reticulo-endothelial system, can have ligands attached to them to target them to tumors and other bad neighborhods, can be lyophilized, sterile filtered, and so on.
     
    The common wisdom is that emulsions cannot be filtered because the droplets are too large, and they cannot be frozen, let alone lyophilized, lest they fall apart. However, this is not necessarily true, as it is possible with enough phospholipids and with high enough pressure to reduce droplet size to well below the 200 nm limit of a sterilizing filter. Even lyophilization is possible if the formulation and lyo cycle are right.
     
    I suspect that the biggest reason emulsions have had such a disappointing career in the injectable drug delivery business is their high start up cost. Especially in these days of tight-fisted venture capitalists paying for much of early drug development, it is much quicker, easier, and cheaper to pull some hydroxypropyl beta cyclodextrin or polysorbate 80 off the shelf than to develop an emulsion. Once something goes into humans, change is hard, and sometimes when we think "it's just for Phase 1," it's still there when the ink is drying on the NDA.
     
    Formulations made with Cremophor or Polysorbate are as messy as those excipients themselves, but the appeal of cyclodextrins is undeniable, and I tend to use them when I can. However, not every solubilization approach works for every molecule, and there are times (eg when treating patients with compromised kidneys) when a cyclodextrin formulation is not appropriate. In times like that I might still want to pull an emulsion out of the pile in the corner, dust it off a bit and try to give it some luster.

    One of the things I occasionally do to add some adventure to my life is to inadvertently consume foods containing peanuts or walnuts, and when I do, I reach for one of the best-known prefilled autoinjector products, the Epi-Pen. One jab into the thigh and 0.3 mg of epinephrine is rapidly making its way into my bloodstream to rescue me from a whole host of anaphylactic symptoms, potentially including the most inconvenient symptom of all, death.
     
    Back in the old days, when I was a kid, my mother would break off the end of an ampule and then syringe out the appropriate amount of epinephrine before manually jabbing me and squirting it in. Being well versed in the use of needles and syringes, I have unsuccessfully attempted to buy vials of epinephrine so I would no longer need to carry the big bulky EpiPens with me, but I am in the minority. For most people pre-filled syringes and autoinjectors are a huge boon, and for good reasons
     
    1. They are convenient for patients (at least for those without Ph.Ds in pharmaceutical sciences) and for practitioners.
     
    2. They substantially reduce the risk of dosing error.
     
    3. They eliminate the risk of contamination while filling syringes.
     
    4. For very expensive drugs, they can actually reduce cost of goods, since less of an overfill is required than for a vial.
     
    5. They can save precious seconds in emergency situations.
     
    6. They can be made to look a lot more stylish than a vial and syringe.
     
    7. In the case of autoinjectors, they can eliminate some of the unpleasantness and anxiety patients might have in poking themselves with a needle. With the Epipen, for example, you don't even don't even see the needle until you pull it out of your thigh, and by that time, it doesn't matter if you freak out.
     
    8. Autoinjectors and, to a lesser extent, pre-filled syringes can be used by patients who have disabilities that make normal syringing difficult or impossible.
     
    All of these things have led to a rapid expansion in the products supplied in prefilled syringes and autoinjectors. In turn, the expanded market has given impetus to new research efforts to improve the design of syringes and autoinjectors and the materials they are made of. A particularly notable example of this innovation is the Unifill® EZMix syringe, a dual chamber syringe containing a liquid chamber and a lyophilized powder chamber. The two chambers are intermixed and the lyophilized powder is reconstituted when the plunger is pushed.
     
    Of course, the story is not all rosy. As with any technology, especially those pushed by the marketing guys with their fast cars and perfect hair, there are drawbacks on the development side:
     
    1. Since prefilled syringes and autoinjectors need to fill the dual role of containing a drug solution and delivering it, there are more contact materials, and these materials are not as easy to optimize for compatibility and low extraction. Hence there is considerably greater potential for compatibility and leachable issues than with drug solutions in vials.
     
    2. Conversely, the contact materials that are important to the function of the device, such as plungers and seals, may be degraded over time by contact with the drug solution. For example, the lubrication between the plunger and the wall of the syringe may leach away, causing the syringe to freeze-up when it is needed.
     
    3. Regardless of how the filled syringes and autoinjectors are sterilized, there are many more potential problems that could occur as the result of sterilization than would be encountered in a glass vial with a grey butyl rubber stopper.
     
    4. Drugs for which the dose is quite variable may not lend themselves to pre-filled systems.
     
    Aside from all this, there is another problem that has come up a few times in the past year with some of our early stage clients, and that is that pre-filled syringe manufacture is still fairly specialized, and it really has not reached the small scale yet. This is typically not a problem but can be if the drug product under development is dependent on some of the attributes of pre-filled syringe. For example one of my recent clients was developing an injectable as a two-vial system, and the accuracy of adding the vial B contents to vial A was quite critical. Since patients would be taking their medicine home, prefilled syringes would have been an ideal solution, but we were unable to identify a sterile products manufacturer with a syringe-filling line willing and able to manufacture as small a batch as we needed.
     
    Unless you are working on a subcutaneous sustained release dosage form of some sort, there is not a lot of glamour in parenterals, and these new multichambered prefilled syringes and autoinjectors add a nice splash of innovation and excitement for us. Even the plain old boring pre-filled syringes play an important role in making healthcare easier and safer for both patients and practitioners. If those device engineers can just come up with an EpiPen that is the size of a Swiss Army knife, we will have made some real progress.

    The transition to electronic manufacturing is now at full tilt due to the technological systems developments that has occurred over the last 25 years, and now, electronic manufacturing is the industry standard for manufacturing companies who must comply with current Good Manufacturing Practices (cGMP). There are countless benefits to making the transition to electronic manufacturing, however, small to mid-sized manufacturing companies and the contact manufacturing organizations which support them continue to resist making the switch. Companies are hesitant to make the change due to the cost and amount of time required to switch from paper to electronic manufacturing systems. Companies also want to ensure that profits will be higher and the return on investment (ROI) will be worth the costs.
     
    Numerous systems are available which can provide ROI for electronic solutions, but it is important to choose a system which is most effective and meets a company's requirements. The return on investment should be considerably greater than the costs needed to solve problems in traditional manufacturing systems. The costs incurred on implementing a new system include the initial prices, maintenance upgrade fees, staff training costs, and the loss of productivity while the new system is installed. Additionally, extra time may be needed to further train staff if employees struggle with the transition away from the paper system.     Writing a speech and presenting it to an audience is an art. And like every art form it doesn't happen overnight. You need to put your time, effort and skills behind it if you want it to work for you.
